粪便幽门螺旋杆菌抗原检测对消化系统肿瘤诊断的意义

摘　　要：目的探讨粪便幽门螺杆菌(helicobacter pylori,Hp)抗原与胃癌和非胃癌消化系统肿瘤的关系。方法将204例消化系统肿瘤患者按肿瘤部位分组并设立健康对照组,采用免疫胶体金法检测各组患者粪便幽门螺杆菌抗原,分析粪便幽门螺杆菌抗原与胃癌和其他各组的关系。结果胃癌组74例,年龄(57.3±11.6)岁;结肠癌组30例,年龄(59.5±12.7)岁;食管癌组35例,年龄(55±9)岁;直肠癌组30例,年龄(57.8±12.3)岁;肝癌组35例,年龄(54.7±11.16)岁和健康对照组80例,年龄(55.4±12.4)岁;粪便幽门螺杆菌抗原阳性率胃癌组(43.24%)均高于健康对照组(1.25%)、结肠癌组(16.61%)、食管癌组(2.86%)、直肠癌组(10%)、肝癌组(5.71%)。结论粪便幽门螺杆菌抗原与胃癌关系极为密切,胶体金法检查Hp抗原简单方便。Objective To study the relation of fecal Helicobacter pylori（Hp） antigen with gastric cancer and gastrointestinal cancers. Methods Two hundred and four patients were divided into gastric cancer group （n=74）, colon cancer group （n=30）, esophageal cancer group （n=35）, rectal cancer group （n=30）, liver cancer group （n=35） and gastrointestinal cancer group （n=130）. Eighty he althy subjects served as a control group. Fecal Hp antigen in different groups was detected by immune colloidal gold assay. Relation of fecal Hp antigen with gastric cancer and gastrointestinal cancers was analyzed. Results Stool Hp antigen was detected in 74 gastric cancer patients, 30 colon cancer patients, 35 esophageal cancer patients, 30 rectal cancer patients, and 35 liver cancer patients with their age being （57.3 ± ll.6）years, （59.5± 12.7）years, （55 ± 9）years, （57.8 ± 12.3）years, and （54.7 ± 11.16）years, respectively. The positive rate of stool Hp antigen was higher in gastric cancer group than in different gastrointestinal cancer groups （43.24% vs 16.61%, 2.86%, 10%, 5.71% ） and in healthy group（43.24% vs 1.25%）. Conclusion Stool Hp antigen is closely related with gastric cancer. Immune colloidal eold assay is a simple method for detectine stool Hn antieen.
