早产儿呼吸窘迫综合征高危因素分析

摘　　要：目的:分析与早产儿呼吸窘迫综合征(RDS)发病相关的高危因素,探讨有效的防治措施。方法:回顾性分析2011年1月1日至2011年12月31日在我院新生儿科住院的1 079例早产儿资料,采用Logistic回归筛选出与RDS相关的因素。结果:(1)早产儿RDS的发生率随着胎龄和出生体重的下降而升高,35～36周早产儿RDS发生率为3.29%,小于28周上升为81.63%;大于2 500 g早产儿RDS发生率仅4.15%,小于1 000 g上升为100%(P<0.05)。(2)二元Logistic回归分析显示:围生期窒息、双胎或多胎、妊高征、胎膜早破为RDS发生的独立危险因素,围生期窒息与RDS的发生最密切(OR依次为15.112、2.140、1.060、0.538,P<0.05)。结论:早产儿RDS的病因复杂,除胎龄和体重外,围生期窒息是早产儿发生RDS的最主要危险因素。Objective： The aim of this study was to asses the potential risk factors of respiratory distress syndrome（RDS） in preterm infants.Methods： One thousand and seventy nine preterm infants admitted to the neonatal intensive care unit（NICU） at Xuzhou Women and Children＇s Health Care Hospital between January 1,2011 and December 31,2011 were studied retrospectively.Logistic regression analysis was used to examine associations between RDS and its potential risk factors.Results：（1） The incidence of RDS increased progressively with decreasing gestational age and birth weight,e.g.from 3.29% at 35~36 weeks up to 81.63% at ≤28 weeks of gestation;from 4.15% in 2 500 g up to 100% 1 000 g of birth weight（P〈0.05）.（2） Perinatal asphyxia,twins or multiple births,maternal hypertension,placental abruption were identified as risk factors associated with the development of RDS and the incidence of RDS especially in immature neonates was highly associated with perinatal asphyxia with an OR（15.112,2.140,1.060,0.538,respectively,P〈0.05）.Conclusions： The findings suggest that the etiology of RDS in preterm infants is multifactoral.In addition to gestational age and weight,the main risk factor of RDS is perinatal asphyxia in preterm infants.
