喉罩与气管插管在小儿全麻气道管理中效果和安全性分析

摘　　要：【目的】比较喉罩与气管插管在小儿全身麻醉气道管理中效果和安全性。【方法】行全身麻醉的患儿120例，随机分为对照组和观察组，每组60例。对照组采用插管通气，观察组采用喉罩通气，比较两组气道管理中的效果和安全性。【结果】观察组置喉罩或插管、拔喉罩或拔管时间点心率（HR）和中心动脉压（MAP）均低于对照组，且差异有显著性（P〈0．01）。观察组副反应发生率低于对照组，且差异有显著性（P〈0．01）。观察组苏醒时间（8．7±4．5）min少于对照组（14．5±5．8）min，且差异有显著性（P〈0．01）。【结论】小儿全身麻醉气道管理中采用喉罩通气对气道损伤小，有利于血流动力学的稳定，且具有苏醒迅速，术后并发症少等优点。[Objective]To compare the efficacy and safety of laryngeal mask vs tracheal intubation for air- way management of pediatric patients with general anesthesia. [Methods]A total of 120 pediatric patients with general anesthesia were divided into observation group and control group with 20 patients in each group. The control group received tracheal intubation, while the observation group was treated with laryngeal mask venti- lation. The efficacy and safety in airway management were compared between two groups. [Results]Heart rate（HR） and central arterial pressure（MAP） in the observation group at the time of placing laryngeal mask or intubation and withdrawal of laryngeal mask or extubation were lower than those in control group, and there were significant differences（ P 〈0.01）. The incidence of side effects in the observation group was lower than that in the control group, and there was significant difference（ P 〈0.01）. The recovery time in observation group was 8.7±4.5min, which was shorter than that in the control group（14.5± 5.8rain）, and there was sig- nificant difference（ P 〈0.01）. [Conclusion]Laryngeal mask applied in airway management of pediatric pa- tients with general anesthesia has minimal airway injury, and is benefit for the hemodynamic stability, and has the advantages such as rapid recovery and fewer postoperative complications.
