摘 要:目的:探讨8周常规康复训练和本体感觉加强训练对膝关节半月板术后患者膝关节本体感觉和肌力的影响,以指导临床康复训练。方法:25名半月板术后患者,依照均衡原则分为常规康复训练组(13人,以肌力和关节活动度训练为主)和本体感觉加强训练组(12人,以本体感觉训练为主)。在8周训练的不同时间点,使用BIODEX PRO 3 SYS测定膝关节主动30°位置觉(术前和术后4、8周)及屈膝30°和60°股四头肌最大等长肌力(术前和术后8周),于术前和术后2、4、8周使用X-SENS测定三维位置觉。结果:8周不同康复训练后,两组患侧、健侧主动30°位置觉较术前均有加强,患侧变化有统计学意义(P=0.022),不同康复处方训练对患侧、健侧主动30°位置觉的影响无差异。8周不同处方训练后两组三维位置觉在4周和8周均有明显改善,不同处方训练对患侧、健侧三维位置觉的影响无差异。8周不同处方训练后两组患侧、健侧30°和60°等长肌力均呈增长趋势,不同处方训练对患侧、健侧30°和60°等长肌力的影响无显著性差异。结论:8周常规康复训练和本体感觉加强训练均能增强膝关节半月板术后患者主动30°位置觉和三维位置觉,但两种训练对本体感觉的影响无显著性差别,可能与其均增强了肌力有关。Objective To compare the influences of 8-week conventional rehabilitation and proprioception strengthen training on the knee joint proprioception and strength of patients with meniseeetomy. Methods Twenty five patients with meniscectomy were randomly divided into two groups: conventional rehabilitation group (n=13 ,mainly underwent muscle strength and motion of joint training) and proprioception strengthen training group (n=12,mainly underwent proprioception training). The active joint sense of position at 30° before,and 4 and 8 weeks after training,isometric strength of quadrieeps femoris at 30° and 60° before and 8 weeks after training, and 3-dimensional sense of position before,and 2 weeks,4 weeks and 8 weeks after training were measured and compared. Results Therewas no significant difference in active joint sense of position, 3-dimensional proprioception and isometric force between two groups (P 〉 0.05). The sense of position at 30° enhanced after 8 weeks training in both groups(P = 0.022) with no significant difference between two groups(P 〉 0.05). Isometric strength of quadriceps femoris at 30° and 60° increased in two groups after 8 weeks training without significant difference between two groups. Conclusion The active sense of position at 30° and three-dimensional sense of position in patients with meniscectomy can be improved by both 8-week conventional rehabilitation or proprioception strengthen training,whereas they reveal insignificant difference between the two groups.
