摘 要:目的探讨磁共振氢质子波谱技术(1H-MRS)在听觉皮层应用的可行性,并进一步就影响检测结果准确性的若干质量控制因素进行讨论。方法采用单体素和多体素两种定位技术对10例健康志愿者右侧初级听皮层行1H-MRS扫描,分别对多体素波谱在听皮层颞平面、颞横回后内侧、颞横回前外侧的代谢物进行测量;单体素作为对照组。测量的主要代谢物包括N-乙酰天门冬氨酸(NAA)、肌酸(Cr)、胆碱(Cho)、谷氨酰胺和谷氨酸(Glx)、γ-氨基丁酸乳酸(GABA)、乳酸(Lac)等等,计算NAA/(Cho+Cr)、Glx/(Cho+Cr)、GABA/(Cho+Cr)峰下面积比值并进行统计学分析。结果多体素技术在听皮层各点的代谢物测量有较大差异,颞平面NAA/(Cho+Cr)比值最高,和对照组相比差异有统计学意义(t=3.932,p=0.001);颞横回后内侧Glx/(Cho+Cr)最高,和对照组相比差异有统计学意义(t=2.354,p=0.044);颞平面GABA/(Cho+Cr)最高,和对照组相比差异有统计学意义(t=3.457,p=0.003)。结论多体素1H-MRS较单体素技术更能准确反映听皮层代谢物含量的变化,设备的质量控制、扫描参数的合理设定以及感兴趣区的科学定位是结果准确可靠的保证。Objective To discuss the feasibility and quality control strategies of proton magnetic resonance spectroscopy(1H-MRS) on auditory cortex. Methods Both single-voxel and multi-voxel technologies are performed iH-MRS scanning on right auditory cortexes of 10 healthy volunteers, metabolites of auditory cortex temporal plane, transverse temporal gyri rear inner and anterolateral are respectively measured with multi-voxel; single-voxel is control group. The main metabolites measured are N-aceytlaspartate(NAA), creatine(Cr), choline(Cho), glutamateJglutamine(Glx), 7-aminobutyric acid lactic acid(GABA), lactic acid(Lac) and so on. To calculated ratio of area under peak for NAA/(Cho4Cr), GIx/(Cho+Cr) and GABA/(Cho-HCr) ratios and make statistic analysis. Results Significant difference is found on chemical substances values with multi-voxel. NAA/(Cho+Cr) in planum temporale is significantly higher than that of single voxel(t=3.932, p=0.001 ), Glx/(Cho+Cr) in rear inner of transverse temporal gyri is significantly higher than that of single voxel(t=--2.354, p=0.044), GABA/(Cho+Cr) in planum temporale is significantly higher than that of single voxel(t=-3.457, p=0.003). Conclusion Multi-voxel 1H-MRS could reflect metabolic character in auditory cortex more unerringly than single-voxel one. Quality control of equipment, reasonable setting of parameters and scientific orientation of interesting region could ensure the accuracy of resalts.
