氨氯地平阿托伐他汀钙对高血压合并血脂异常患者血管功能的影响

摘　　要：目的研究复方氨氯地平阿托伐他汀钙对高血压合并高脂血症患者血管功能的影响。方法入选180例高血压合并血脂异常患者,按随机双盲法分为对照组90例(氨氯地平5 mg/d)和试验组90例(复方氨氯地平阿托伐他汀钙1片/d),治疗12个月。测定治疗前后患者的血压、血糖、血脂、颈动脉内中膜厚度(IMT)、肱动脉内径及血浆内皮素1(ET-1)及一氧化氮(NO)水平。结果与氨氯地平相比,复方氨氯地平阿托伐他汀钙可延缓颈动脉IMT的进展[(-0.17±0.07)mm比(-0.11±0.05)mm,P<0.05],并提高患者NO水平[(35.0±6.5)μmol/L比(19.5±6.4)μmol/L,P<0.05]和肱动脉扩张能力(10.1%±3.1%比8.8%±3.2%,P<0.05),降低ET-1水平[(-31.8±6.5)ng/L比(-21.8±7.1)ng/L,P<0.05];但二者的降压作用差异无统计学意义[(-29±7.5/-16.5±7.5)mm Hg比(-26±9.0/-13.5±9.0)mm Hg,P>0.05]。结论复方氨氯地平阿托伐他汀钙较氨氯地平血管保护功能更强。Objective To investigate the effects of Amlodipine Besylate and Atorvastatin Calcium Tablets on vascular function in patients with hypertension accompanied with dyslipidemia. Methods One hundred and eighty patients with hypertension and dyslipidemia were randomly divided into two groups： Amlodipine Besylate Group （n=90） and Amlodipine Besylate and Atorvastatin Calcium Group （n = 90 ）. Blood pressure, glucose, lipids profile, carotid artery IMT, plasma ET and NO levels were measured and compared between two groups. Results Compared to Amlodipine Besylate Group, blood pressure had no significant difference [（ -3.9±1.0）/（-2.2±1.0） mmHgvs. （ -3.4±1.2）/（-1.8±1.2） mmHg, P〈0. 01], but plasma ET-1 level I（-31.8±6.5） ng/Lvs. （ -21.8±7.1） ng/L, P〈0.05J was significantly decreased, and carotid artery IMT [ （ - 0. 17 ±0. 07 ）mm vs. （ -0. 11 ± 0. 05 ） mm, P 〈 0. 05 J was reduced more in Amlodipine Besylate and Atorvastatin Calcium Group. The increase of plasma NO level [（35.0 ±6.5）μmol/L vs. （19.5±6.4） μmol/L, P 〈0.051 and dilation of brachial artery E （10. 1%± 3. 1% ） vs. （8.8% ±3.2% ）, P 〈 0. 051 was more significant in Amlodipine Besylate and Atorvastatin Calcium Group than that in Amlodipine Besylate Group. Conclusions Amlodipine Besylate and Atorvastatin Calcium Tablets has protective effects on vascular function in patients with hypertension accompanied with dyslipidemia.
