补肾活血祛风法治疗脾肾气虚兼痰瘀型Ⅳ、Ⅴ期糖尿病肾病临床研究

摘　　要：目的:观察补肾活血祛风方对脾肾气虚兼痰瘀型Ⅳ、Ⅴ期糖尿病肾病的疗效。方法:采用随机、平行、对照的方法将入选的病例分为治疗组24例、对照组21例。在基础治疗的基础上,对照组口服缬沙坦,治疗组口服缬沙坦联合中药补肾活血祛风方,疗程均为12周,观察相关指标的改变。结果:两组治疗前后空腹血糖、HbA1c、收缩压、舒张压、Tg,LDL及TC治疗前后改变量比较均无显著性差异(P>0.05);治疗组可改善中医证候,降低蛋白尿及SCr,此作用优于对照组。结论:补肾活血祛风方与ARB类药联用能够显著缓解DN临床症状,降低蛋白尿,降低SCr,保护肾功能;此作用明显优于单用ARB类药。Objective：To observe the effect of Kidney-Replenishing Blood-Promoting and Pathogenic Wind-Dispelling Recipe in the treatment of diabetic nephropathy （DN）in the stage of IVand V of spleen and kidney Qi deficiency with phlegm and blood stasis intermingled syndrome. Methods ： The DN patients of spleen and kidney Qi deficiency with phlegm and blood stasis intermin- gled syndrome, were randomly divided into 2 groups of treatment group （ 24 cases） and control group （21 cases）. All of cases were given healthy education, diabetic control, antidiabetics, antihypertensive agent and blood cholesterol reduce if necessary;then the control group was given Valsartan;and the treatment group was given Valsartan and Kidney-Replenishing Blood-Promoting and Pathogenic Wind-Dispelling Recipe. The therapeutic courses of the two groups were both 12 weeks, and the changes of the correlated index after treatment were observed. Results ： The levels of blood-fasting sugar, HbA1 c, SP, DP, TG, LDL and the changes of TC had no difference between treatment group and control group（P 〉0. 05）. The total effective rate of symptoms in treatment group was higher than that in control group. Moreover,the clinical study showed that Kidney-Replenishing Blood-Promoting and Patho- genic Wind-Dispelling Recipe could cut down total urinary protein, decrease SCr, which is significantly different between the treat- ment group and controlled group. Conclusion：Kidney-Replenishing Blood-Promoting and Pathogenic Wind-Dispelling Recipe com- bining with Angiotensin-Converting Enzyme Inhibitor（ARB） could relieve symptoms, cut down total urinary protein, decrease SCr and protect kidney of DN patients in stage 1V and V, and the effect is more noticeable better than that of ARB.
