仰卧位颈椎定点拔伸推顶法治疗神经根型颈椎病的疗效观察

摘　　要：目的:观察仰卧位颈椎定点拔伸推顶法治疗神经根型颈椎病的临床疗效及分析。方法:将临床诊断为神经根型颈椎病的病人随机分成治疗组和对照组,分别采用①理筋手法+定点拔伸推顶法,②理筋手法+颈椎牵引。采用"颈椎病治疗成绩评分表"进行评分,计算改善率,进行疗效观察。结果:共收治病例181例,其中治疗组96例,对照组85例。两组治疗前评分差异无统计学意义(t=0.75,P>0.05),治疗1个疗程后两组患者颈椎病治疗成绩评分都增加,两组与治疗前评分比较差异有非常显著性意义(P<0.01);治疗后治疗组评分、治愈率、总有效率与对照组比较差异有显著性意义(P<0.05),治疗组疗效明显优于对照组。结论:仰卧位颈椎定点拔伸推顶法是根据神经根型颈椎病的发病机制,结合颈椎的解剖结构特点而形成,能纠正紊乱的颈椎椎体及小关节,临床操作简单,舒适安全有效,易于推广。Objective ： To observe and evaluate the clinical effect of cervical local-point traction pushing manipulation on supine position for the treatment of cervical spondylotic radiculopathy. Methods ： Patients with cervical spondylotic radiculopathy were randomly allocated to manipulation group and control group, separately treated with massage and cervical local-point traction pushing manipulation, massage and traction for cervical spine. According to scores of Assessment Scale for Cervical Spondylosis before and after treatment, the improvement rate was accounted and the effect was observed. Results：There were 181 patients of cervical spondylotic radiculopathy, the manipulation group had 96 cases, and the control group 85 cases. Scores of the patients in the manipulation and control group were similar before treatment （t = 0.75,P 〉 0.05）. It had obvious increase compared scores of two groups after one course of treatment than that before therapy（P 〈 0.01 ） ;there was a significant disparity between two groups in scores, cure rate and total effective rate after treatment （P 〈 0.05 ）, which showed that the therapeutic effect of the manipulation group was superior to that of the control group. Conclusion：This manipulation is made according to pathogenesis of cervical spondylotic radiculopathy and combining anatomic structure features of cervical vertebrae, which has achieved effect through adjusting vertebra displacement after treatment. It is easy to spread due to simple and safe operation.
