摘 要:目的探讨医院真菌感染的临床分布及其危险因素,为预防控制医院真菌感染提供依据。方法对2010年1月-2011年12月389例真菌感染患者的临床资料进行统计分析。结果深部真菌感染已成为院内获得性感染棘手的严重问题,其感染对象多数为有严重基础疾病的患者,真菌感染部位以呼吸道为主占56.8%,其次为泌尿系统和血液感染多见;最常见的真菌是白色假丝酵母菌占54.8%,其次为热带假丝酵母菌占9.3%;免疫力低下患者、长期应用广谱抗菌药物、激素、化疗放疗及侵入性操作等是感染的危险因素。结论应重视医院内真菌感染的预防控制工作,通过合理应用抗菌药物、减少不必要侵入性操作、加强真菌的培养检测、指导临床合理用药,从而有效预防与控制医院真菌感染。OBJECTIVE To investigate the clinical distribution of deep fungal infections and explore the risk factors so as to provide basis for the basis for the prevention and control of nosocomial fungal infections.METHODS From Jan 2010 to Jan 2011,the clinical data of 389 cases of patients with fungal infections were statistically analyzedRESULTS The deep fungal infections have become a severe hospital-acquired infection which was very difficult to treat,which was very common in the patients with underlying diseases.The respiratory tract was the most common infection site,accounting for 56.8%,followed by the urinary system and the blood.The Candida albicans was the most prevalent species of fungi causing the deep fungal infections,accounting for 54.8%,followed by the Candida tropical(9.3%).The causes of these infections were highly associated with the risk factors such as immuno-compromised,extended use of broad spectrum antibiotics,hormones,chemotherapy and radiotherapy as well as the invasive operation.CONCLUSION It is necessary to attach great importance to the prevention and control of the hospital-acquired fungal infections by reasonably using antibiotics,reducing the unnecessary invasive operations,intensifying the culture and detection of fungi,and guiding the reasonable use of antibiotics so as to effectively prevent and control the fungal infections.
