持续负压密封引流技术治疗骨科感染伤口的观察与护理

摘　　要：目的观察持续负压密封引流技术(VSD)治疗骨科感染伤口的临床效果及护理特点,为临床治疗及护理提供依据。方法选取医院2010年4月-2012年3月收治并应用VSD患者30例为治疗组,选取同期利用常规换药引流的20例患者为对照组;观察两组患者经治疗及护理后创面生长及感染情况。结果治疗组30例应用VSD装置7～11d,平均9.2d,进行病原菌培养选用抗菌药物,感染均得到控制,其中23例患者在7～10d引流后创面肉芽新鲜,行Ⅱ期缝合后愈合,7例患者进行2～3次VSD创面恢复新鲜,予以手术缝合;对照组20例中,10例经换药给予抗菌药物后创面感染得到控制,10例患者伤口经治疗15d愈合后,给予手术缝合。结论持续VSD能够充分引流坏死物质及渗液,对伤口的愈合及防治并发症具有重要作用。OBJECTIVE To observe the clinical efficacy of continuous vacuum sealing drainage（VSD） technique in the treatment of infected wounds in the department of orthopedics and analyze the nursing features so as to provide basis for the clinical treatment and nursing.METHODS Totally 30 patients who were enrolled from Apr 2010 to Mar 2012 and were treated with VSD were selected as the treatment group,and in the same period 20 patients who received the routine dressing change drainage were set as the control group.The wound growth and the infection status of the two groups of patients were observed after the treatment and nursing.RESULTS The 30 cases in the treatment group were treated with VSD for 7-11 day with the average of 9.2 days,all the cases with infections have been under control by use of antibiotics on the basis of the bacterial culture.There were 23 cases who suffered from the granulation fresh 7-10 days after the drainage and healed after Ⅱ stage suture and 7 cases whose wounds recovered fresh after being treated with VSD for 2 or 3 times and were given the suture.Of the 20 cases in the control group,there were 10 cases with the wound infections under control after the routine dressing change and 10 cases with the wounds cured after a 15-day treatment,who were then given the suture.CONCLUSION Continuous VSD can fully drain the necrotic material and exudate,which has great significance in the wound healing and the prevention and treatment of complications.
