小儿心内科住院患儿抗菌药物使用调查

摘　　要：目的调查小儿心内科心脏病住院患儿抗菌药物的使用情况及合理性,为临床合理使用抗菌药物提供指导依据。方法对67例小儿心内科各种心脏病住院患儿抗菌药物的使用情况进行调查。结果抗菌药物的使用率为52.24%;单用占76.19%,单用加二联占90.48%;主要抗菌药物为头孢孟多、头孢唑肟、拉氧头孢、阿奇霉素、头孢呋辛、头孢哌酮/舒巴坦、亚胺培南;用药途径以静脉注射为主;预防用药在住院患者中的总使用率为7.46%,为导管介入患儿;其余均为治疗性用药;细菌标本送检率为74.29%。结论抗菌药物使用偏多,起点偏高;细菌培养的送检率对于抗菌药物的合理使用非常重要;治疗性用药指针需严格掌握;针对多药耐药菌株感染应采取联合用药,并应足量足程;需进一步加强临床医师的抗菌药物知识更新和公众普及教育。OBJECTIVE To investigate the clinical application of antimicrobial agents of the hospitalized children in department of pediatric cardiology and its rationality so as to provide basis for the clinical reasonable use of antibiotics.METHODS A total of 67 hospitalized children with various cardiovascular diseases were enrolled in the study,the application of the antimicrobial agents was investigated.RESULTS The utilization rate of antibiotics was 52.24%;the single application rate was 76.19%,the single plus two-drug use rate was 90.48%;the most frequently used antimicrobial agents were cefamandole,ceftizoxime,latamoxef,azithromycin,cefuroximem,cefopcrazone-sulbactam,and imipenem;the intravenous injection was the main drug delivery approach;the utilization rate of prophylactic antibiotics was 7.46% among the hospitalized patients who were the catheter-intervened patients;the rest were therapeutic use of antibiotics;the submission rate of bacterial specimens was 74.29%.CONCLUSION The utilization rate of antibiotics is high with the high starting point;the submission rate of the bacterial culture is crucial to the reasonable use of antibiotics;it is necessary to grasp the guidelines for the therapeutic use of antibiotics;the combination use of antibiotics should be performed in line with the multidrug-resistant strains with sufficient quantities and course.
