摘  要:目的分析首都医科大学附属北京世纪坛医院患者泌尿系深部真菌感染病原菌的分布及其耐药性,为临床合理选用抗真菌药物提供依据。方法收集北京世纪坛医院2009年1月-2010年12月尿液样本中分离的真菌,采用科玛嘉培养基及VITEK-2Compact鉴定系统进行菌株鉴定,ATB Fungus 3进行药物MIC测定,卡方检验进行统计学分析。结果共分离出各类酵母样真菌217株,白色假丝酵母菌66株,占30.4%,热带假丝酵母菌57株,占26.3%,光滑假丝酵母菌53株,占24.4%,近平滑假丝酵母菌20株,占9.2%;分离的真菌对5-氟胞嘧啶、两性霉素B敏感性好,对氟康唑耐药率为4.6%与伏立康唑的8.8%差异无统计学意义,对氟康唑耐药率为7.4%与伊曲康唑的14.3%差异有统计学意义(χ2=5.37,P<0.05)。结论尿液标本真菌感染以白色假丝酵母菌、热带假丝酵母菌、光滑假丝酵母菌、近平滑假丝酵母菌为主,对5-氟胞嘧啶、两性霉素B、氟康唑、伏立康唑敏感性较好。OBJECTIVE To analyze the distribution and antibiotic resistance of pathogenic fungi causing urinary tract infections of Shijitan Hospital so as to facilitate the appropriate selection of antimicrobial agents in clinical practice.METHODS The pathogenic fungi isolated from urine samples between Jan 2009 to Dec 2010 in Shijitan Hospital were collected and identified by CHROMaga media and VITEK-2 Compact system,the drug susceptibility testing was performed with ATB Fungus 3 and the chi square test was used for statistics analysis.RESULTS Of totally 217 strains of various species of Candida,there were 66(30.4%) strains of Candida albicans,57(26.3%) strains of C.tropicalis,53(24.4%) strains of C.glabrata and 20(9.2%) strains of C.parapsilosis;the fungi isolates were very sensitive to 5-flucytosine and amphotericin B,there was no statistical difference between the drug resistance rates to fluconazole(4.6%) and voriconazole(8.8%),and there was statistical difference between the drug resistance rates to itraconazole(14.3%) to fluconazole(7.4%)(χ2=5.37,P0.05).CONCLUSION C.albicans,C.tropicalis,C.glabrata and C.parapsilosis are predominant pathogenic fungi causing urinary tract infections,which are very sensitive to 5-flucytosine,amphotericin B,fluconazole and voriconazole.
