脑卒中吞咽障碍患者吞咽功能改善前后的呼吸功能变化

摘　　要：目的观察脑卒中吞咽障碍患者吞咽功能改善前后的呼吸功能变化。方法将40例脑卒中后咽期吞咽障碍患者按随机数字表法分为治疗组和对照组,每组20例。治疗组采用头控技术结合呼吸训练及姿势调整,同时配合电刺激治疗及摄食训练;对照组仅采用电刺激治疗及摄食训练。每项治疗均为2次.d-1,共治疗20 d。治疗前后2组采用洼田氏饮水试验进行吞咽障碍评定,通过肺活量、最长呼气时间及每分钟呼吸次数评定呼吸功能。结果 2组组内治疗前、后洼田氏饮水试验评定结果比较,差异有统计学意义(P<0.05),且治疗组优于对照组(P<0.05);治疗组治疗前、后呼吸功能结果比较,差异有统计学意义(P<0.05),对照组组治疗前、后呼吸功能结果比较,差异无统计学意义(P>0.05)。结论头控技术结合呼吸训练及姿势调整能明显改善卒中后吞咽障碍及呼吸功能,且随着患者呼吸功能好转,吞咽功能改善明显,患者生活质量明显提高。Objective To observe the changes in respiratory function before and after improvement in deglutition function in patients with pharyngeal dysphagia after stroke. Methods Forty patients with pharyngeal dysphagia after stroke were randomly divided into two groups, with 20 patients in each group. Patients in treatment group were treated with head control technology, breathing training, posture adjustment, electrical stimulation and eating training. Patients in control group were only given electri- cal stimulation and eating training. Each treatment modality was administrated twice a day for 20 days. Post-stroke dysphagia was evaluated using drinking test and respiratory function was assessed by vital capacity, maximum expiratory time and respiratory frequency before and after treatment. Results Both groups showed significant improvement in swallowing function after treatment（P〈0.05 ）, and the im- provement in treatment group remarkably increased compared with control group（P〈0.05 ）. Breathing function in treatment group was obviously improved after treatment（P〈0.05）, but no significant improv- ement was found in control group（P〉0.05）. Conclusion Head control technology combined with bre- athing quality training and posture adjustment can improve swallowing and breathing functions to increase the of life in patients with pharyngeal dysphagia after stroke.
