双侧上尿路梗阻致急性肾衰竭的血液透析治疗

摘　　要：目的总结双侧上尿路梗阻致急性肾衰竭患者血液透析治疗的临床经验,提高诊治水平。方法回顾性分析2008年1月至2011年12月安徽医科大学第一附属医院泌尿外科收治的107例双侧上尿路梗阻致急性肾衰竭行血液透析治疗患者临床资料。结果 107例患者中86例采用IHD治疗,12例采用EDD治疗,9例采用CVVH治疗。89例患者具备手术指征,81例手术后肾功能恢复至正常,术后肾功能完全恢复率为91%(81/89)。18例患者因基础疾病及高龄不能手术,规律透析维持治疗。结论围手术期血液透析可以迅速改善肾功能,为手术创造条件,应根据患者病情不同选择不同的透析方式。及时手术解除梗阻,有助于肾功能的恢复。Objective To summarize the clinical experience of the hemodialysis among patients with acute renal failure caused by bilateral upper urinary tract obstruction and to improve the diagnosis and treatment.Methods A retrospective study of 107 patients（from Jan.2008 to Dec.2011） identified as acute renal failure in the hospital and treated with hemodialysis was performed.Results Eighty-six cases were treated with IHD treatment,12 cases with EDD,and 9 cases with CVVH treatment.Among 89 patients with surgical indications,totally 81 patients regained normal renal function after treatment,showing a recovery rate of 91%.Eighteen patients were treated with regular dialysis due to underlying diseases and old age.Conclusion Perioperative hemodialysis can rapidly improve renal function and patient condition for the surgery.The selection of the type of dialysis should be based on individual condition.Timely surgery to remove the obstruction can contribute to the recovery of renal function.
