原发性高血压患者腹腔镜胆囊切除术内皮素水平的研究

摘　　要：目的　了解腹腔镜胆囊切除术 (laparoscopiccholecystectomy,LC)对原发性高血压患者内皮素 (endothelinET)水平的影响。方法　通过前瞻性研究 ,采用放射免疫法 ,对行LC术和开腹胆囊切除的 37例Ⅱ、Ⅲ期原发性高血压胆囊结石患者及 76例单纯胆石症患者 ,分别于术前、术中、术后测定其血浆ET水平。结果　高血压组术前ET值高于单纯胆石症患者 ,但差异无显著意义 (P >0 0 5 ) ;高血压LC组术中ET值稍高于术前 ,差异无显著意义 (P >0 0 5 ) ,术后ET值显著低于术前、术中 (P <0 0 1) ,高血压开腹组术中ET值稍高于术前 ,差异无显著意义 (P >0 0 5 ) ,术后ET值低于术前 ,但差异无显著意义 (P >0 0 5 ) ;单纯胆石症LC组术中ET值稍高于术前 ,差异无显著意义 (P >0 0 5 ) ,术后显著低于术前、术中 (P <0 0 1) ,开腹组术中ET值稍高于术前 ,差异无显著意义 (P >0 0 5 ) ,术后显著低于术前 (P≤ 0 0 5 ) ;高血压LC组ET值术中与术前及术后与术前差值稍高于单纯胆石症组 ,但无统计学意义 (P >0 0 5 )。结论　LC术对原发性高血压患者ET水平无明显影响。Objective To investigate the effect of LC on plasma ET level in patients with essential hypertension (EH). Methods Plasma ET level of 37 stage Ⅱ、Ⅲ EH patients with gallbladder stones and 76 normotensive gallbladder stone patients was measured by radioimmunological methods before and after undergoing LC or cholecystectomy.Results The preoperative plasma ET level of EH patients was higher than that of normotensive patients (P>0 05); The intraoperative ET level of EH patients undergoing LC was higher than that preoperatively(P>0 05), though the postoperative ET level was significantly lower than that intraoperatively and preoperatively(P<0 01). The intraoperative ET level of EH patients undergoing open cholecystectomy(OC) was higher than that preoperatively(P>0 05),and postoperative ET level was lower than that preoperatively(P>0 05). The intraoperative ET level of normotensive patients undergoing LC was higher than that preoperatively(P>0 05), and postoperative ET level was significantly lower than those preoperatively and intraoperatively(P<0 01). The intraoperative ET level of normotensive patients undergoing OC was higher than that preoperatively(P>0 05), and postoperative ET level was lower than that preoperatively (P≤0 05).Conclusion Neither LC nor OC has an effect on ET level in EH patients undergoing LC and OC.
