老年患者肺结核两种治疗方案的比较

摘　　要：目的探讨一种新的治疗方案治疗>70岁老年患者初治肺结核的有效性与耐受性,为老年初治肺结核提供良好治疗方案。方法 105例>70岁的老年初治肺结核患者随机分为治疗组与对照组,治疗组52例,对照组53例,治疗组采用力克肺疾、利福喷丁、莫西沙星新的治疗方案治疗,对照组采用标准短程治疗方案治疗。结果治疗方案治疗组的完成率为90.4%,对照组为54.7%,治疗组明显高于对照组(P<0.01);治疗组疗程1个月和2个月时的痰菌阴转率分别为76.6%和89.4%,对照组分别为75.9%和86.2%,差异无统计学意义;疗程结束时治疗组肺部病灶吸收率和好转率分别为74.5%和91.5%,对照组分别为72.4%和89.7%,差异无统计学意义。结论新的治疗方案对>70岁老年患者初治肺结核具有良好耐受性及满意的疗效。OBJECTIVE To investigate the efficacy and tolerance of a new chemotherapy regimen for the initialtreatment of pulmonary tuberculosis in patients aged more than 70 years so as to provide better treatment program for initical treatment of tuberculosis in the elclely. METHODS A total of 105 initial pulmonary tuberculosis patientsaged more than 70 years were randomly divided into the treatment group and the control group,with 52 cases in the treatment group,and 53 cases in the control group. The treatment group was treated with a new chemotherapy regimen containing pasiniazid,rifapentin,and moxifloxacin,and the control group was treated with standard short-course chemotherapy regimen. RESULTS The chemotherapy completion rate of treatment group was significantly higher than that of control group （90.4% vs. 54.7%, P 〈0.01）;the sputum conversion rates at the first and second month of the treatment group were 76.6% and 89.4% ,separately,while the control group were 75.9% and86.2% （P ~ O. 05）~the pulmonary lesions apparent absorption rate and lesion improvement rate at the end of treatment of the treatment group were 74.5% and 91.5 %, respectively,while the control group were 72.4% and89.7%, the differences were not statistically significant. CONCLUSION The new chemotherapy regimen can achieve good tolerance and satisfied efficacy in the initial treatment of pulmonary tuberculosis in the patients aged more than 70 years.
