摘 要:目的探讨SjMAg、KLH及rSj32检测日本血吸虫病人的特异性和敏感性及疗效考核价值。方法采用ELISA法,用SjMAg、KLH及rSj32及SEA检测治疗前及治疗后不同时期血吸虫病人血清中的特异性IgG、IgG4及正常人和肺吸虫、肝吸虫、囊虫病人血清中的非特异抗体。结果SjMAg、KLH及rSj32的敏感性和特异性与SEA比较差异均无显著性(均P>0.05);用SjMAg和KLH分别检测治疗后12个月血清中IgG和IgG4的阴转率均显著高于SEA(P<0.01)、检测治疗后24个月血清中IgG4的阴转率均达97.6%。结论SjMAg、KLH及rSj32具有与SEA相似的诊断价值和较高的疗效考核价值;特异性IgG4可能系一种疗效敏感抗体,可望成为疗效考核评价指标。Objective To elucidate the specificity, sensitivity and treatment efficacy of SjMAg, KLH and/or rSj32 in detecting specific IgG and IgG4 antibodies in sera of patients with schistosomiasis japonica before and after treatment. Methods SjMAg, KLH and/or rSj32 were used in ELISA to determine the specific IgG and IgG4 in sera of patients with schistosomiasis japonica before and after treatment, and the cross-reactivity was tested with sera of normal persons and patients with paragonimiasis or clonorchiasis or cysticercosis. Results SjMAg, KLH and rSj32 were as sensitive and specific as SEA in diagnosis of schistosomiasis japonica. The respective negative conversion rates of IgG and IgG, detected by SjMAg, KLH and rSj32 12 months after treatment were significantly higher than that by SEA. The negative conversion rates of IgG. detected by either SjMAg or KLH 24 months after treatment were similar as 97.6%. Conclusion SjMAg, KLH and rSj32 showed the same value as SEA in diagnosing schistosomiasis japonica and higher value in assessing therapeutic efficacy. The specific IgG4 may be a short-duration antibody which can indicate successful chemotherapy of patients with Schistosoma japonicun infection.
