摘 要:目的观察柴胡注射液对内毒素大鼠弥散性血管内凝血(DIC)多器官损伤及磷酸化ERK 1/2(p-ERK 1/2)表达的影响。方法利用内毒素(脂多糖,LPS)复制大鼠DIC多器官损伤模型,将大鼠按给药不同随机分为柴胡低、高剂量组(分别腹腔注射0.2,0.4 g/kg柴胡注射液)、模型组和对照组。并于注射后5 h后取血,利用全自动血凝仪及生化仪检测活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)、乳酸脱氢酶(LDH)、谷丙转氨酶(ALT)、谷草转氨酶(AST)、尿素(BUN)及肌酐(Cr)等指标,光镜下观察心肝肾组织病理学改变,免疫组化检测p-ERK 1/2在肝组织中的分布。结果模型组APTT、LDH、ALT、AST、BUN及Cr均较对照组明显延长或升高(P<0.01),FIB较对照组显著降低(P<0.01),并出现典型DIC的病理学特征,免疫组化显示肝组织阳性细胞明显增高。柴胡注射液处理后各生化指标均显著改善(P<0.01),病理学可见各脏器损伤减轻,免疫组化显示肝组织阳性细胞密度减低。结论柴胡能够减轻内毒素DIC多器官损伤,其机制可能与ERK1/2信号通路的阻断有关。Objective To examine the effect of injection bupleuri on multiple organ injury (MOI) and expression of p-ERK1/2 in endotoxin-inducing disseminated inravascular coagulation (DIC) rat. Methods DIC rat model of MOI was established with LPS (6 mg/kg) by intraperitoneal injection. SD rats were randomly divided into bupleuri (0. 2, 0.4 g/kg), model and control groups. The blood sample was taken after 5 h. Activated partial thromboplastin time ( APTT ), fibrin ( FIB ), lactate dehydrogenase ( LDH ), aspartate aminotransferase (ALT), aspartate aminotransferase( AST), blood urea nitragen(BUN) and Creatininie (Cr) were detected by MC-2000 coagulation autoanalyzer and Olympus biochemical autoanalyzer. Heart, liver, kidney were taken out and pathology change of the tissue were observed, p-ERK1/2 expression of liver was detected by immunohistochemieal in the end of test. Results Compared with control group, APTT, LDH, ALT, AST, BUN and Cr were obviously extended or increased (P 〈0. 01), while FIB was reduced (P 〈0. 01) in model group. Heart, liver and kidney biopsy showed vascular engorgement and the positive cell was apparently increased in liver. All the biochemical indicators ameliorated among bupleuri than those of model groups (P 〈0.01 ), while pathological changes were alleviated in heart, liver and kidney and the positive cell was decreased in liver. Conclusions Injection of bupleuri can attenuate multiple organ dysfunction in the pathogenesis in endotoxin-inducing DIC rat and the mechanism possibly involves ERK1/2 inhibition.
