重组链激酶治疗急性脑梗死出凝血功能的动态变化

摘　　要：目的　动态观察 10 0万IU重组链激酶 (r -SK)治疗早期急性脑梗死患者 2 4h内出凝血功能变化。方法　 6 0例入选患者随机分为治疗组和对照组 ,测定治疗前后的凝血酶原时间 (PT)、部分凝血活酶时间(APTT)、凝血酶时间 (TT)、血浆纤维蛋白原 (Fg)、抗凝血酶 -Ⅲ含量 (AT -Ⅲ∶Ag)和抗凝血酶 -Ⅲ活性 (AT -Ⅲ∶A) ,同时观察治疗效果和出血情况。结果　治疗组用药后凝血功能减弱、Fg和AT -Ⅲ∶A下降。治疗组疗效明显优于对照组 ,发生出血者较未出血者出凝血功能的下降更为明显。结论　 10 0× 10 4 IU的r -SK引起患者出凝血功能下降的幅度和持续时间有限。该剂量r -SK治疗早期急性脑梗死患者基本是安全有效的。Objective To observe the changes of hemostatic function in 24 hours in patients with acute cerebral infarction treated with 1,000,000IU of recombinant streptokinase(r-SK).Method Sixty selected patients were divided into r-SK group and control group and their PT,APTT,TT,Fg,AT-Ⅲ:Ag and AT-Ⅲ:A were surveyed.Results After r-SK treatment,the patients' PT,TT and APTT prolonged,Fg and AT-Ⅲ:A decreased and all of them had statistical significances compared with controls.The changes lasted for 24 hours.Conclusion Although the dosage of r-SK could affect the hemostatic function of patients,the range and duration are limited.From this point of hemostasis,this dosage of r-SK is safe in treating early stage of acute cerebral infarction.
