摘 要:目的:探讨不同强度电针、热灸样刺激"后三里"穴对小鼠痛阈的影响及其感受器受体机制。方法:清洁级小鼠20只,分为香草酸瞬时受体亚型1基因敲除(TRPV 1-/-)组和C 57BL/6同源野生型对照组,每组10只。在"后三里"穴进行电针(强度分别为0.3mA、1.0mA或3.0mA,持续20min)或热灸样刺激(强度分别为38℃、43℃或46℃,持续10min)干预,在电针和热灸样刺激干预结束即刻测量小鼠同位和异位机械痛阈和热痛阈,并在0.3mA、3.0mA电针和38℃、46℃热灸样刺激结束5min后再次测量小鼠同位和异位痛阈。结果:①与对照组相比,TRPV 1-/-组的基础热痛阈显著升高(P<0.001)。②1.0mA、3.0mA电针和43℃、46℃热灸样刺激干预均可引起C 57BL/6小鼠同位和异位机械痛阈和热痛阈显著升高(P<0.05,P<0.01,P<0.001),而0.3mA电针和38℃热灸样刺激干预仅能引起C 57BL/6小鼠同位机械痛阈和热痛阈显著升高(P<0.05,P<0.01)。3.0mA电针可显著提高TRPV 1-/-小鼠同位和异位的痛阈(P<0.05,P<0.01)。1.0mA电针可显著提高TRPV 1-/-小鼠同位痛阈和异位机械痛阈(P<0.05,P<0.01)。0.3mA电针和43℃、46℃热灸样刺激干预仅能引起TRPV 1-/-小鼠同位痛阈的显著升高(P<0.05,P<0.01),而38℃热灸样刺激仅可引起TRPV 1-/-小鼠同位机械痛阈的升高(P<0.05)。③与对照组相比,电针或热灸样刺激预处理对TRPV 1-/-组小鼠痛阈的升高效应均有不同程度的减弱。④电针或热灸样刺激干预结束5min后,两组小鼠痛阈基本恢复至干预前水平。结论:电针和热灸样刺激均可对痛阈有一定的升高效应,说明电针和热灸样刺激具有预防性抗痛效应,而且TRPV 1参与了这一镇痛效应。Objective To observe the effect of electroacupuncture (EA) and moxibustion-like thermal stimulation with different temperatures at "Zusanli"(ST 36) on pain reactions in transient receptor potential vanilloid type 1 knock-out (TRPV 1-/-) mice so as to studY its mechanisms underlying analgesia. Methods Ten C 57 BL/6 mice and 10 TRPV 1-/- mice were respec- tively divided into control and TRPV 1-/- groups. EA (0.3 mA, 1.0 mA,3.0 mA) or thermal water-bath (38 13, 43 ℃, 46 ℃) stimulation was applied to the right ST 36 for 20 min and 10 min, respectively. Mechanical and thermal pain thresholds of the bila- teral paws were detected before and immediately after stimulation, and 5 min after EA stimulation at 0.3 mA, 3.0 mA and thermal water-bath stimulation at 38 ℃, 46 ℃ ,separately. Results ① The thermal threshold was significantly higher in TRPV 1-/- mice than in O 57 BL/6 mice (P〈0. 001). ② In C 57 BL/6 mice, both homolateral and contralateral mechanical and thermal pain thresholds were significantly increased following 1.0 mA, 3.0 mA EA, and 43 ℃ and 46 ℃ thermal stimulation of the right Zusanli (ST 36) ( P〈0. 05, P〈0. 01, P〈0. 001) ;while only homolateral mechanical and thermal pain thresholds were increased after0.3 mA EA and 38 ~3 thermal stimulation of ST36 (P〈0. 05, P〈0. 01). In TRPV 1-/- mice group, both homolate-ral and contra- lateral mechanical and thermal pain thresholds were significantly increased by 3.0 mA EA stimulation of ST 36 ( P〈0. 05, P〈0. 01). Bilateral mechanical thresholds and homolateral thermal threshold were significantly increased by 1.0 mA EA stimula- tion of ST 36 (P〈0. 05, P〈0. 01), while only homolateral mechanical and thermal pain thresholds were increased by 0.3 mA EA, and 43 ℃ and 46 ℃ thermal stimulation (P〈0.05, P〈0. 01).
