平板运动试验心率恢复环在冠心病诊断中的意义

摘　　要：目的 :评价平板运动试验心率恢复环 ( HRRL)在冠心病 ( CAD)诊断中的价值。方法 :分析 88例受检者之 HRRL 与冠状动脉造影结果的关系 ,计算 HRRL 诊断 CAD的敏感性及特异性 ,并与常用的 ST段标准进行比较。结果 :平板运动试验 ST段标准 ,HRRL标准诊断 CAD的敏感性分别为 6 0 .5 %和 83.7% ;特异性分别为82 .2 %和 73.3%。 HRRL标准和 ST段标准之 Kappa值为 0 .5 1。结论 :与常用的 ST段标准进行比较 ,HRRL在CAD诊断中敏感性较高 。Objective:To evaluate the clinical value of heart rate recovery loop (HRRL) during treadmill exercise test in the detection of coronary artery disease(CAD). Method:Data of coronary artery angiogram and treadmill exercise electrocardiogram of 88 cases were analyzed. The sensitivities and specificities of HRRL and ST segment criteria were compared.Result:HRRL had four types, type A, B, C and D. Type A and type B (HRRL criteria) were used for the diagnosis of CAD. The sensitivities of ST segment criteria and HRRL were 60.5 % and 83.7 % respectively.The specificities of ST segment criteria and HRRL were 82.2 % and 73.3 % respectively. The reproducibility of ST segment criteria and HRRL was good with Kappa value of 0.51 .Conclusion:Compared with traditional ST segment criteria, HRRL had a higher sensitivity and a lower specificity in the detection of CAD.
