斑马导丝引导下筋膜扩张器治疗尿道下裂术后尿道狭窄

摘　　要：目的:探讨采用斑马导丝引导下筋膜扩张器治疗尿道下裂术后尿道狭窄的临床疗效。方法:2005年4月～2011年10月采用斑马导丝引导下筋膜扩张器治疗尿道下裂术后尿道狭窄患者18例。结果:15例采用该法成功施行尿道扩张,历经2～6次不等,扩张后均未发生尿路感染、附睾炎、尿道大量出血、尿道穿破或假道形成等严重并发症;3例尿道瘢痕狭窄严重或尿道成角而未能置入斑马导丝,择期改行尿道成形术。15例获得4～18个月随访,均排尿通畅,尿线粗而有力;6例术后2～6个月行尿流率检查,最大尿流率均大于20ml/s。结论:采用斑马导丝引导下筋膜扩张器治疗尿道下裂术后尿道狭窄操作简单,安全性高,临床应用疗效良好,值得推广。Objective： Investigate the clinical effect of urethral stricture after hypospadias surgery which was treated by fascial dilator using zebra guidewire. Method： Analysised the clinical date of 18 urethral strictures after hypospadias surgrey that were treated by fascial dilator using zebra guidewire from April 2005 to October 2011. Result： Fifty of them were been successfully cured after 2-6 times urethral sounding, without serious complication such as urinary infection, bleeding, bladder broken, false passage or epididymitis. 15 children were followed by 4- 18 months, who all pissed easily and smoothly, and the urinary streams were thickness and powerful. The Qmax- es were bigger than 20 ml/s in 6 children after the operation for 2-6 months. Conclusion： It is a easier handling, sa- ,fer and more effective method to cure urethral stricture after hypospadias surgerys. So the clinical therapeutic effect was satisfactory, and it should be extended.
