渐进性屈指支具在外伤性掌指关节僵硬治疗中的应用

摘　　要：目的探讨渐进性屈指支具配合系统康复治疗外伤性掌指关节僵硬的疗效。方法配合系统康复治疗，采用渐进性屈指支具治疗外伤性掌指关节僵硬23例85指，治疗前、后分别测量患指掌指关节的被动关节活动度（passive range of motion，PROM）、主动关节活动度（active range of motion，AROM），并进行上肢功能评定（disability of the arm，shoulder and hand，DASH）。结果全部患者康复治疗时间平均为（6．74±1．54）周。测量患指掌指关节：PROM治疗前平均（24．45±17．15）°，治疗后平均（57．87±15．66）°；AROM治疗前平均（10．25±10．38）°，治疗后平均（21．05±16．07）°。按DASH评分：治疗前平均（41．13±14．08），治疗后平均（26．13±12．07）。患者对疗效满意度达91．3％。结论渐进性屈指支具配合系统康复治疗可以有效地改善外伤性掌指关节僵硬的主、被动关节活动度，进而改善上肢功能。Objective To evaluate the treatment effectiveness of progressive finger flexion orthosis and hand therapy in the management of traumatic metacarpophalangeal joint stiffness. Methods Progressive finger flexion orthosis combined with systematic hand therapy interventions were used to treat traumatic metacarpephalangeal joint stiffness in 85 fingers from 23 cases. Before and after the interventions, passive range of motion （PROM）, active range of motion （AROM） and disability of the arm, shoulder and hand （DASH） score were measured. Results Patients received （6.74±1.54）weeks＇ treatment. PROM was improved from （24.45±17.15）° to （57.87± 15.66）°; AROM increased from （10.25± 10.38）° to （21.05 ± 16.07）°; DASH score decreased from （41.13 ± 14.08） to （26.13 ± 12.07）. There were significant differences in these three measurements before and after the therapy. The satisfactory rate of patients reached 91.3 %. Condusion Combined with serial hand therapy treatments, progressive finger flexion orthosis significantly enhances PROM and AROM of metacarpephalangeal joints with post-traumatic stiffness, and further improves function of the upper extremity.
