急性痛风性关节炎的中医研究进展

摘　　要：随着痛风的发病率逐渐增高,中医药治疗急性痛风优势引起关注,本文通过查阅古籍与近十年的临床文献,从病因病机、辨证论治、常用验方与成药、针灸、中药外敷与注射等方面综述了目前中医药治疗急性痛风所取得的研究进展,认为急性痛风属于中医学之痹证;其病因病机为肾虚,湿邪阻滞,热毒攻于手足;以湿热蕴结为主要证型;治疗以清热利湿为主,方多为四妙散加减;针灸治疗以循经取穴针刺和局部围刺放血为主;中药外敷多以大黄、黄柏等清热泻火解毒药敷于关节痛处;中医药治疗急性痛风疗效确切,副作用少,值得临床广泛应用。As the incidence rate of gout increases gradually,the advantage of TCM（Traditional Chinese Medicine） therapy of treating acute gout has aroused attention.Through the review of ancient books and clinic literature,this paper concluded the achievements of TCM treatment on acute gout from aspects of etiology and pathogenesis,syndrome differentiation and treatment,commonly used prescriptions based on experience,acupuncture and moxibustion,external application and injection of Chinese medicine,etc.This review finds acute gout belongs to Bi syndrome in TCM;the etiology and pathogenesis of which is kidney deficiency,the block of dampness evil,toxic heat attacking the fingers and toes;the main syndrome is stagnation of damp-heat;the therapeutic method is clearing heat and removing dampness;the commonly used prescription is modified ＂Simiao Powder.;acupuncture and moxibustion therapy include acupuncturing with selecting acupoint along channels and bleeding around the painful area;external application of Chinese medicine on painful joints mostly chooses clearing heat medicine such as Chinese rhubarb and Phellodendron Bark,etc.TCM treatment which has good efficacy and fewer side effects is worth of wide clinical application.
