纱垫填塞法治疗腹部手术大出血疗效分析

摘　　要：目的:探讨纱垫填塞法治疗腹部手术大出血的疗效。方法:回顾性分析我院1996年10月至2011年12月收治的腹部手术大出血采用纱垫填塞法止血的9例患者的临床资料。其中肝破裂合并肝后大血管出血4例,肝破裂合并门脉、胆管破裂并弥漫性血管内凝血1例,腹膜后肿瘤切除术中大出血1例,肝硬化脾切除术后大出血1例,直肠癌骶前大出血2例。结果:7例患者止血成功,均在3-7d拔除填塞纱条,患者无术后再出血及腹腔脓肿形成。2例患者因多器官功能衰竭死亡。结论:纱垫填塞法治疗腹部手术大出血是一种简便、快速、有效的方法。Objective： To explore the effect of gauze packing for bleeding of abdominal surgery.Method： Retrospectively analyzed the clinical datas of 9 patients received gauze packing for bleeding of abdominal surgery from Oct.1996 to Dec.2011.There were 4 cases of liver rupture with hepatic vascular hemorrhage,1 case of liver rupture combined with rupture of portal vein and bile duct and disseminated intravascular coagulation,1 case of resection of the tumor in the retroperitoneal bleeding,1 case of cirrhosis of the liver after splenectomy bleeding and 2 cases of rectal cancer presacral bleeding.Result： Seven cases of patients received successful hemostasis,and were removed of packing gauze in 3-7 days.There were no side effects like postoperative bleeding and abdominal abscess formation in these patients.2 patients died due to multiple organ failure.Conclusion： Gauze packing method is a simple,fast and effective way for bleeding of abdominal surgery.
