摘 要:目的:探讨孕期合并甲状腺疾病及其诊治时间对新生儿的影响。方法:选择2009年2月~2012年3月36例妊娠合并甲状腺疾病孕妇所产新生儿,根据孕期甲状腺疾病类型分为甲状腺功能减退症(甲减组)、孕母亚临床甲状腺功能低下(亚甲减组)及孕母甲状腺功能亢进(甲亢组)根据甲状腺疾病确诊时间分为:孕前及孕早期发现甲状腺疾病组(早发现组)、孕中期及孕晚期发现甲状腺疾病组(晚发现组),调查分析各组新生儿的转归。结果:孕母甲减组、亚甲减组及甲亢组三组间比较,新生儿在早产率、出生体重、头围、身长和甲状腺功能异常发生率各方面差异均无统计学意义(P>0.05)。孕母甲状腺疾病早发现组新生儿出生体重高于晚发现组,但甲状腺功能异常发生率却低于晚发现组,差异均有统计学意义(P<0.05),而在早产率、头围和身长等方面两组差异无统计学意义(P>0.05)。所有甲状腺功能异常患儿经观察治疗,预后均较好。结论:孕期合并甲状腺疾病会对新生儿造成一定影响,应早尽早诊断和早期治疗。Objective:To explore the effects of pregnancy combined with thyroid diseases,diagnostic time,and curative time on neonates.Methods:Thirty-six neonates born by pregnant women with thyroid diseases from February 2009 to March 2012 were selected,then they were divided into hypothyroidism group,subclinical hypothyroidism group,and hyperthyroidism group according to the types of thyroid diseases during gestational period;meanwhile,the neonates were divided into early onset group(thyroid diseases were detected during pregestational period and the first trimester of pregnancy) and late onset group(thyroid diseases were detected during the second and third trimesters of pregnancy);the outcomes in different groups were investigated and analyzed.Results:There was no statistically significant difference in neonatal incidence of premature birth,birth weight,head circumstance,body height,and incidence of thyroid dysfunction among hypothyroidism group,subclinical hypothyroidism group,and hyperthyroidism group(P0.05).Neonatal birth weight in early onset group was higher than that in late onset group,but the incidence of thyroid dysfunction in early onset group was lower than that in late onset group,there were statistically significant differences(P0.05).There was no statistically significant difference in incidence of premature birth,head circumstance,and body height between the two groups(P0.05).After treatment,the prognosis of all the neonates with thyroid dysfunction was well.Conclusion:Pregnancy combined with thyroid diseases has a certain impact on neonates,early diagnosis and early treatment should be conducted.
