针刺治疗中风后尿潴留的系统评价

摘　　要：目的:评价针刺治疗中风后尿潴留的有效性。方法:按照循证医学的要求,检索国内相关医学数据库,以随机临床对照实验和半随机对照实验为纳入标准,治疗组以针刺治疗为主,对照组以尿潴留常规治疗为主,以中风后尿潴留治愈率为评价指标,由两名评价者独立提取资料并进行方法学质量评估,最终纳入8篇(684例)文献,并对其采用ReMan4.2.10软件进行Meta分析。结果:治愈率汇总OR值为3.06,95%可信区间CI为[2.19,4.27];治疗组疗效优于对照组(p<0.05),用漏斗图分析发表性偏倚,图形不对称,存在发表性偏倚。结论:针刺治疗中风后尿潴留较常规疗法疗效好,但是由于纳入研究数量和质量上的缺陷,有必要进一步提高研究以获得更高效的证据。Objective：To evaluate the therapeutic effect of acupuncture for treatment of Urinary retention after stroke.Methods：According to the requirements of evidence-based medicine,we retrieved the relative medical database in china,and clinically randomized controlled trials（RCTs） and quasi-RCTs were used as enrolled criteria,and the treatment group was treated with the main therapy of acupuncture,and the control group with the main therapy of conventional treatment of urinary retention in Western Medicine.The curative rate of urinary retention after stroke was used as evaluation index.Dates were extracted and Methodological quality evaluated by two reviewers independently.There were 8 Literatures（684 cases）enrolled in the last,which were conducted for Meta analysis by ReMan4.2.10.Results：The total OR was 3.06 with 95% CI [2.19,4.27] of the curative rate in the 8 studies,the curative effect in the acupuncture group was superior to the conventional treatment group（p0.05）.Funnel plot was used to analyze the possible existence of publication bias,and the Funnel plot was asymmetric which showed the possible existence of publication bias existed.Conclusion：Acupuncture is better than the conventional treatment in the treatment of urinary retention after stroke.There are defects in quality and quantities of trials,so further studies are necessary to perform in order to get more efficient evidence.
