胃左静脉分流指数在食管静脉破裂出血预测中的价值

摘　　要：目的:评价胃左静脉分流指数在食管静脉破裂出血预测中的价值。方法:根据有无出血史将患者分为出血组(30例)、未出血组(22例)及健康对照组(15例),共三组。根据患者结扎的情况,15例出血患者又分为结扎前后两组进行研究。结果:出血组胃左静脉分流指数明显高于未出血组及对照组(P<0.001)。当胃左静脉分流指数截断值定于0.12时,其预测出血的敏感性、特异性及准确性分别高达93.7%、90.0%及92.3%。同时结扎后胃左静脉分流指数较结扎前明显降低(P<0.001)。结论:胃左静脉分流指数具有较高的预测价值。Objective:To evaluate left gastric vein shunt index (LGVSI)for predicting esophageal variceal bleeding.Methods:Thirty variceal patients with previous hemorrhage(group Ⅰ),22 variceal patients without previous hemorrhage(group Ⅱ)and 15 control were examined with color Doppler sonography.Fifteen patients in group Ⅰ were also examined after endoscopic variceal ligation(EVL).The LGVSI was measured and predictive value were studied.Results:The LGVSI in the group Ⅰ was significantly higher than that in group Ⅱ and in control( P <0 001).When the best cut off value of the LGVSI was 0 12,the diagnostic sensitivity,specificity and accuracy rate were 93 7%,90 0% and 92 3%,respectively.The LGVSI after EVL was significantly lower than before( P <0 001).Conclusion:Results indicate LGVSI is highly sensitive and specific in predicting esophageal variceal bleeding.
