肾盂癌中EpCAM和E-Cadherin的表达及临床意义

摘　　要：目的探讨EpCAM和E-Cadherin的表达与肾盂癌病理分级和临床分期的关系。方法应用免疫组织化学SP法检测76例不同级别肾盂癌和50例癌旁正常组织中EpCAM和E-Cadherin的表达。结果 EpCAM在肾盂癌阳性表达率为60.5%(46/76),明显高于癌旁组织27.6%(21/76),差异具有统计学意义(P<0.05);EpCAM的表达与肾盂癌不同临床分期、病理分级、远处转移及继发膀胱肿瘤相关(P<0.05);EpCAM与E-Cadherin在肾盂癌中表达呈负相关(r=-0.425,P<0.01)。结论EpCAM及E-Cadherin的表达变化与肾盂癌的发生发展、浸润转移有良好的相关性,同步检测二者在肾盂癌组织中的表达并综合分析两者之间的关系对评价肾盂癌的侵袭转移能力判断具有一定价值。Objective To investigate the prognostic value of EpCAM and E-Cadhefin in urothelial carcinoma of renal pelvis. Methods Ep- CAM and E-Cadherin were, analyzed by immunohistnchemistry using monoclonal antibodies in 50 cases of normal mueosa and 76 cases of urothelial eareinoma of rrenal pelvis. Results The positive rate of EpCAM in urothelM carcinoma of rrenal pelvis was 60.5% ,which was significantly higher titan in para-caneerous mueosa of 27.6%. The expression of EpCAM was significantly related with the differential clinical stage, pathological degree, distant metastasis and bladder tumor recurrence （all P 〈 0.05 ）. The expression of E-Cadhefin was significantly rre- lated with the differential clinical stage, pathological degree and distant metastasis （ all P 〈 0.05 ）, There, was negative correlation between the expression of EpCAM and E-Cadhefin in umthelial carcinoma of rrenal pelvis （r =-0.425, P 〈 0.01 ）. Conclusion The expression EpCAM and E-Cadherin are. highly rrelated to occurrence, development, infiltration and metastasis of urothelial carcinoma of renal pelvis, and evaluation of the two proteins could he a very applicable guidance for the diagnosis of metastasis and invasion of urothelial carcinoma of renal pelvis.
