围关节胫骨近端锁定钢板系统在胫骨粉碎性骨折治疗中的应用

摘　　要：目的探讨围关节胫骨近端锁定钢板内固定术治疗胫骨粉碎性骨折的临床疗效。方法回顾分析某院自2009年1月～2010年6月,采用胫骨近端锁定钢板内固定法治疗32例胫骨粉碎性骨折患者,选择小腿胫前外侧弧形切口,通过锁定钢板楔形尾端插入式固定骨折部位,32例均获得解剖复位并骨性愈合。结果 32例患者获得12个月的随访,患肢皮缘无坏死、切口无感染、无大血管损伤,内固定无松动或断裂,膝关节无卡压及退行性改变,骨折处均骨性愈合,临床治愈率达100%。结论围关节胫骨近端锁定钢板内固定系统,具有高度稳定性及创伤小等优点,可加速骨折处骨性愈合,减少关节术后并发症的发生。OBJECTIVE To explore the clinical effect of locking plate fixation of periarticular proximal tibia in treatment on com- minuted fractures. METHODS Retrospectively analyzed 32 cases of comminuted fractures treated by locking plate fixation of pe- riarticular proximal tibia from January 2009 to June 2010. They were also obtained anatomical reposition and bony healing with the choice of anterolateral incisions on tibial calf, through the locking plate wedge end plug-in fixed fracture sites. RESULTS 32 patients were received 12 months follow-up, no necrosis, no infection, no damaged blood vessel, no loosed fracture fixa- tion, and no knee press-fitting and degenerative change, all of them were bony healing, clinical cure rate was 100%.CON- CLUSION The locking plate fixation of periarticular proximal tibia systems is with the higher stability and smaller trauma. It can accelerate the bony union, and reduce postoperative complications.
