尿沉渣分析仪Sysmex UF-500i在快速筛查尿路感染中的应用价值

摘　　要：目的:以中段尿定量细菌培养为金标准,评价SysmexUF-500i尿沉渣分析仪(简称UF-500i)细菌定量计数参数在快速筛查尿路感染(Urinary tractinfection,UTI)中的应用价值,为临床经验用药提供参考。方法:采集260份中段尿标本,以尿液细菌培养菌落计数阳性为诊断UTI金标准,比较分析UF-500i细菌定量计数结果,并计算其诊断UTI的敏感性、特异性、阴性及阳性预测值、假阳性及假阴性率和诊断准确率。结果:260份尿液标本中细菌培养阳性87例,占33.4%;以UF-500i细菌定量计数临界值3750/μl为筛查UTI的诊断临界值,UF-500i细菌定量计数与定量培养结果比较差异无统计学意义(P>0.05),其诊断UTI的敏感性为85.1%,特异性为91.3%,阳性预计值为87.0%,阴性预计值为92.5%,假阳性率为8.7%,假阴性率为14.9%,准确率为90.7%。在尿培养阳性的前两位致病菌中,两种方法定量计数大肠埃希菌和克雷伯菌属细菌的相关性分别为0.990和0.989。结论:UF-500i操作简便、快速和准确度高,在一定范围内可作为UTI的快速筛检指标和指导临床经验用药。Objective：To compare the UF-500i urine sedimentation analyze （UF-500i） with urine bacterial culture in screening urinary tract infection （UTI）,and to provide a reference for the clinical experience of drug use.Method：260 fresh midstream urine were collected and examined by cultures and counts of bacterial colonies and UF-500i analysis.The results between the two methods were compared,and then the sensitivity,specificity,negative/positive predictive value,false positive/negative rate and accuracy were calculated.Result：The positive rate of urine culture was 87 patients （33.4%） and there was no significant difference between UF-500i bacterial quantitative counts and urine bacterial cultures （P0.05） when the bacterial quantitative counts of 3750/μL was used to screen UTI diagnosis threshold.The sensitivity of UF-500i was 85.1%,specificity was 91.3%,positive predict value was 87.0%,negative predict value was 92.5%,false positive rate was 8.7%,false negative rate was 14.9%,and accuracy was 90.7% in screening UTI.The correlation of intestinal Escherichia coli between the two methods was 0.990 and that of Klebsiella bacteria was 0.989 for the first two pathogens in the positive culture.Conclusion：UF-500i has a character of simple,rapid,and high accuracy,which may be used to screen UTI and provide a reference for the clinical experience of drug use.
