青年肺结核患者就诊延迟预后评价的统计学分析

摘　　要：目的探讨青年肺结核患者就诊延迟影响因素及其预后评价,为促进结核病的预防和控制提供依据。方法采取现况调查方法,以2004年8月至2007年12月间在德州市第二人民医院结核科住院治疗的青年肺结核患者作为研究对象,利用自制调查表收集相关资料,应用logistic回归分析及χ2检验及秩和检验等统计学方法处理数据。结果资料完整者400例。青年肺结核就诊延迟的主要影响因素是出现不适后的决定及知道结核病防治医疗机构(P<0.01)。知道结核病防治机构是保护因素(OR值<1)。两种方法分析结果仍有差别。在其预后评价中,就诊延迟与病情严重程度的相关性有统计学意义(P<0.05),巩固期治疗结束时,痰菌阴转率、病灶吸收患者比例及症状减轻患者比例均降低(P<0.01)。结论就诊延迟的青年肺结核患者病情加重,且治疗效果差,更易造成结核病的传播,并严重危害公众健康,从而加重家庭和社会的经济负担。为预防和控制青年肺结核,提高早期发现率,应加大结核病防治知识的宣传力度,尤其应以在校初中学生作为重点宣传对象。因延迟定义中的界值不同,所以各种研究资料无法进行比较。应采用国际上可接受的、国内外可比较的定义,对肺结核患者发现延迟情况进行更加深入的研究,为我国结核病控制规划提供科学依据。Objective To study the patient delay and the prognosis in the youth patients with pulmonary tuberculosis,for evidence in preventing and controlling tuberculosis.Methods A cross-sectional descriptive survey was conducted in the youth inpatients of pulmonary tuberculosis.The data were analyzed with SPSS13.0 and logistic regression,Chi-square test and rank sum test.Results There were 400 patients with complete data.Results Shows that the factors associated with patient delay are the determination of patients after uncomfortable and knowing tuberculosis prevention and cure organization（P0.01）.Two methods of analysis results are different.In the evaluation of the prognosis,patient delay and the severity of the correlation was statistically significant（P0.05）.The end of a period of consolidation treatment,sputum conversion rate,the proportion of lesions and symptoms in patients with reduced absorption of the proportion of patients were lower（P0.01）.Conclusion Young TB treatment the prognosis of patients delayed not only due to severe illness leading to increased treatment costs,and serious harm to public health.To improve the early detection rate,it is very important to put emphasis on aggrandizing propaganda for the junior middle school students with various methods.Recommended that future should be internationally acceptable,the definition of comparable domestic and foreign,for tuberculosis patients found to delay the case more in-depth research.
