短期胰岛素泵强化治疗对初诊2型糖尿病患者炎性因子及血糖血脂的影响

摘　　要：目的探讨胰岛素泵强化治疗对初诊2型糖尿病患者胰岛功能的保护作用。方法初诊2型糖尿病患者44例随机分为两组,分别给予胰岛素泵强化治疗(CSII组)和多次皮下胰岛素注射(MSII组)治疗2周。比较两组治疗前后空腹血糖(FPG)、餐后2h血糖(2hPG)、糖化血红蛋白(HbA1c)、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、胰岛素抵抗指数(HOMA-IR)、C反应蛋白(CRP)、肿瘤坏死因子α(TNF-α)水平。结果 MSII组和CSII组治疗后FPG、2 hPG、HbA1c、TC、TG、LDL-C、HDL-C、HOMA-IR、CRP、TNF-α均较治疗前明显下降(P<0.05)。CSII组治疗前后变化量明显高于MSII组(P<0.05)。结论胰岛素泵短期强化治疗不仅能平稳降糖,还具有降脂、改善氧化应激及减少炎症的作用,从而达到改善β细胞功能及胰岛素抵抗的目的。Objective Through the observation short-term insulin pump therapy intensified（CSII） and multiple subcutaneous insulin injection（MSII）,before and after blood sugar,blood fat,inflammation factors,insulin resistance index change,this paper discusses the insulin pump first visit to patients with type 2 diabetes mellitus beta cell protection.Methods First visit in patients with type 2 diabetes in 44 patients randomly divided into two groups,namely give insulin pump（CSII group） and multiple subcutaneous insulin injection（MSII group） treatment 2 weeks.Compared between the two groups before and after treatment fasting plasma glucose（FPG）,postprandial 2h blood sugar（2HPG）,glycated hemoglobin（HbA1c）,total cholesterol（TC）,triglyceride（TG）,low density lipoprotein cholesterol（LDL）-C）,high-density lipoprotein cholesterol（HDL-C）,insulin resistance index（HOMA-IR）,c-reactive protein（CRP）,tumor necrosis factor alpha（TNF alpha） level.Results MSII group and CSII group after treatment FPG,2 hPG,HbA1c,TC,TG,LDL-C,HDL-C,HOMA-IR and CRP,TNF alpha than before treatment decreased obviously（P 0.05）.CSII group variation before and after treatment significantly higher than MSII group（P 0.05）.Conclusion Insulin pump short-term intensie treatment not only has the stationary hypoglycemic effect,still have fall fat,improve oxidative stress and reduce the role of inflammation,so it can improve ss-cell function and the role of insulin resistance.
