三种椎间植骨方法在腰椎后路椎间融合的应用分析

摘　　要：目的探讨椎间隙单纯植骨、椎间融合器植骨及椎间融合器内外联合植骨3种椎间隙植骨方法,在腰椎间盘突出症后路手术中椎间融合疗效的差异。方法 36例46个椎间隙接受椎间融合内固定手术,其中10例共14个椎间隙实施椎间盘摘除椎间单纯植骨内固定术,12例16个椎间隙实施椎间融合器置入术,14例16个椎间隙实施椎间融合器内外联合植骨内固定术。摄X线片测量施术节段椎间隙高度变化,进行比较研究。结果随访18～24个月,单纯植骨组术后3个月时椎间隙高度丢失率达13.7%;椎间融合器组术后12～18个月高度丢失率较高,占总丢失量的50.0%。椎间融合器内外联合椎间隙植骨结合了单纯椎间植骨及单纯椎间融合器植骨两者的优点。术后椎间隙高度丢失无明显高峰期,18个月丢失率为7.53%。结论椎间融合器内外联合植骨在腰椎间盘突出症后路手术进行椎间融合治疗中,在维持椎间隙高度、利于椎间骨性愈合方面具有明显优势。Objective To explore the difference of curative effect of three kinds of intervertebral space bone grafting method in the LIDP of PLIF by merely bone grafting of intervertebral space, merely bone grafting for intervertebral fusion device internal and intervertebral fusion device the internal and external combined grafting. Methods 36 patients with 46 vertebral clearance accepted intervertebral fusion and internal fixation surgery, including 10 cases of 14 intervertebral space implementation of the merely bone grafting of intervertebral space combination internal fixation, 12 cases of 16 intervertebral space implementation of merely bone grafting for intervertebral fusion device internal, 14 cases of 16 vertebral clearance implementation of intervertebral fusion device the internal and external combined grafting. Disturbing X ray film measurement applied art segment vertebral clear- ance height change,comparative study was conducted. Results Following up a case by regular visits to 18 -24 months, merely bone grafting group began march vertebral clearance height when dropping 13.7% ;intervertebral fusion device group of 12 to 18 months after dropping height was higher, accounting for 50.0% of the total amount of loss. Intervertebral fusion device the internal and external combined graftsing group vertebral clearance height loss no obvious peak, 18 months for dropping 7.53%. Conclu- sion Intervertebral fusion device the internal and external combined grafting in maintaining the vertebral clearance height and beneficial to the intervertebral osseous healing had obvious advantages.
