中青年无性生活勃起功能障碍患者的原因分析及个性化治疗

摘　　要：目的分析中青年无性生活勃起功能障碍（ED—NS）患者原因并制定治疗策略。方法2010年2月至12月因ED—NS就诊患者68例。采用实时勃起功能检测、血清睾酮检测、血糖测定和阴茎多普勒超声检查分析ED原因。对患者进行充分的性教育，予个性化治疗。采用自制简化ED-NS问卷表SIEF—NS对治疗后的效果进行评估。结果68例ED．NS患者中诊断为心理性ED42例，睾酮低下，诊断为内分泌性ED7例，伴有糖尿病11例，伴有高血压8例。经过个性化治疗4周后，有47例SIEF—NS评分提高〉50％，治疗前后SIEF—NS评分分别为（11．2±2．5）、（19．8±3．9），差异具统计学意义（P〈0．01），有效率为69．1％。结论采用简化SIEF-NS可快速评价ED．NS患者勃起功能状态，中青年ED-NS经过个性化治疗后，勃起功能明显改善。Objective To analyze the causes of no sexual life patients with erectile dysfunction and make individualized therapeutic strategy. Methods A total of 68 ED-NS patients from Feb to Dec 2011 were enrolled in the study. The Self-estimation Index of erectile function-No sexual life （SIEF-NS）, Rigiscan, serum testosterone or penile Doppler were used to evaluate the causes of ED-NS and the effects of individualized treatment. Results Of 68 ED-NS patients, 42 cases were diagnosed as psychological ED, 7 cases as hypogonadism ED, 11 cases asdiabetes mellitus ED and 8 cases as vascular ED. The individualized treatment significantly enhanced the efficacy variables. The score of SIEF-NS was significantly increased after individualized treatment （11.2±2.5 vs 19.8±3.9, P〈0.01）. The overall sal- vage rate was 69.1%. Conclusion The SIEF-NS score may be a useful method for the evaluation of ED-NS patients. The individualized treatment might improve the erectile function of patients with ED-NS effectly.
