乙型肝炎病毒外膜大蛋白检测在恩替卡韦抗病毒治疗中的临床意义

摘　　要：目的探讨慢性乙型肝炎(CHB)患者在用恩替卡韦抗病毒治疗过程中血清HBV表面大蛋白(HBV-LP)水平变化的临床意义。方法检测40例CHB患者在用恩替卡韦抗病毒治疗的0、12、24、36、48、72、96周血清中的HBV-LP、HBV DNA、HBeAg水平。结果治疗过程中,HBVDNA载量对数值及HBV-LP吸光度(A)值均下降,且呈正相关(r=0.989,P<0.01)。不同治疗时期HBV DNA阴性患者的HBV-LP A值与HBV DNA阳性患者比较,差异较大。HBV DNA阳性组的HBV-LP A值均值为1.478。以HBV-LP A值≥1.5为界值时,其与抗病毒治疗效果明显相关(P<0.01)。结论在抗病毒治疗过程中,血清HBV-LP与HBV DNA有较好的相关性,可作为新的判断病毒复制的生物指标。Objective To evaluate the significance of detecting serum hepatitis B virus large surface protein（HBV-LP） during entecavir antiviral therapy.Methods A total of 40 patients with chronic hepatitis B（CHB） was treated with entecavir once daily for 96 weeks.Serum HBV-LP,HBV DNA and HBeAg were detected at the times of 0,12,24,36,48,72,96 weeks.Results During treatment,the logarithm of HBV DNA copies/ml and A value of HBV-LP were all reduced and correlated with each other（r=0.989,P0.01）.The mean A value of HBV-LP was 1.478.Taking≥1.5 as the critical A value of HBV-LP,it was closely related with antivirus efficacy of entecavir therapy（P0.01）.Conclusion Serum HBV-LP is correlated with HBV DNA in the process of antiviral treatment and can be used as a candidate biomarker for evaluating hepatitis B virus infection.A value of HBV-LP≥1.5 indicates that the HBV DNA is supposed to be positive.
