摘  要:目的探讨骨盆正位片对婴儿发育性髋关节脱位(DDH)的诊断价值。方法回顾性分析80例6个月以下临床疑诊DDH的婴儿骨盆平片资料,测量每个髋关节的AI(髋臼指数)、h值(股骨颈上端外侧与Y线的垂直距离)、f值(股骨颈上端内侧凸点与坐骨支相交点的距离),采用单因素方差分析探讨相关数值与髋关节脱位类型的关系。结果本组80例160个髋关节。46个正常髋关节(NH):AI为(24.65±3.11)度,h值为(8.60±2.20)cm,f值为(3.80±1.34)cm;48个发育不良髋(DH):AI为(33.63±3.87)度,h值为(8.13±1.73)cm,f值为(4.61±1.53)cm;30个半脱位髋(SH):AI为(33.87±5.07)度,h值为(7.90±1.82)cm,f值为(6.01±2.38)cm;36个全脱位髋(FDH):AI为(34.50±5.38)度,h值为(4.46±2.61)cm,f值为(7.20±2.75)cm。AI在正常髋与发育不良、全脱位、半脱位间具有统计学差异(P<0.01)。h值在全脱位与正常髋、发育不良、半脱位间具有统计学差异(P<0.01)。SH和FDH的f值明显高于NH(P<0.01)。AI与h值呈负相关(r=-0.315,P<0.05),而与f值呈正相关(r=0.601,P<0.01)。结论骨盆平片对6个月以下婴儿DDH的早期诊断具有重要价值。Objective To investigate the value of pelvis plane film in diagnosing developmental dislocation of the hip(DDH) in infants.Methods Pelvic plane films were taken in 80 infants(six months old,160 hips) with suspected DDH.The acetabular index(AI),f value and h value were measured.Results The values of AI,h value and f value in 46 normal hips(NH) were(24.65±3.11) degrees,(8.60±2.20) cm,and(3.80±1.34) cm,respectively,which in 48 dysplasia hips(DH) were(33.63±3.87) degrees,(8.13±1.73) cm,and(4.61±1.53) cm,in 30 subluxation hips(SH) were(33.87±5.07) degrees,(7.90±1.82) cm,and(6.01±2.38) cm,and in 36 full dislocation hips(FDH) were(34.50±5.38) degrees,(4.46±2.61) cm,and(7.20±2.75) cm.The difference of AI between NH and DH,SH and FDH was significant(P0.01).The difference of h value between FDH and NH,DH and SH was significant(P0.01).The f values of SH and FDH were higher than those of NH(P0.01).AI was negatively correlated to h value(r=-0.315,P0.05),but positively correlated to f value(r=0.601,P0.01).Conclusion Pelvis plane film is valuable in the early diagnosis of DDH in infants.
