C型股骨远端骨折手术内固定疗效分析

摘　　要：目的探讨钢板与逆行交锁髓内钉内固定治疗C型股骨远端骨折的临床疗效。方法 55例股骨远端骨折行手术内固定治疗,其中钢板螺钉固定27例,逆行交锁髓内钉固定28例。观察两组术后骨折愈合时间和HSS膝关节功能评分变化。结果钢板组骨折愈合时间3～10个月,平均(4.8±1.0)个月;髓内钉组骨折愈合时间3～18个月,平均(5.0±1.1)个月,两组骨折愈合时间比较差异无统计学意义(P>0.05)。钢板组功能评分(89.7±11.6)分,优20例,良4例,可2例,差1例,优良率88.9%;髓内钉组(89.3±12.8)分,优21例,良4例,可2例,差1例,优良率89.3%,钢板组术后膝关节HSS评分与髓内钉组比较差异无统计学意义(P>0.05)。结论钢板螺钉内固定治疗股骨远端骨折疗效与逆行交锁髓内钉内固定疗效相当,均能取得良好效果。Objective To compare the clinical and functional outcomes of plate-screw system and retrograde interlocking intramedullary nail for treatment of distal femoral fractures （type C）. Methods A retrospective study was carried out in 55 cases of type C distal femur fractures. In all cases, 27 fractures were fixed with plate-screw system,28 fractures with retrograde interlocking intramedullary nail. Results The bone union was achieved within 3-10 months （4.8±1.0 months on average） with plate-screw system, and within 3-18 months （5.0± 1.1 months on average） with retrograde interlocking intrameduUary nail （P 〉 0.05）. The mean HSS score of the plate group was （89.7±11.6）,and the nail group was （89.3±12.8） （P 〉0.05）. Conclusion The type C distal femoral fractures fixed with the plate-screw system and the retrograde interlocking intrameduUary nail could obtain curative satisfactory functional results.
