摘 要:目的通过钢板螺钉固定人尸体胫骨标本的生物力学测试分析,为微创经皮钢板内固定治疗胫骨骨折提供理论依据。方法选取成人尸体胫骨湿标本40根,随机分成8组,测试载荷-应变指标并进行统计学分析。结果垂直压缩时垂直方向压力-应变第4组(14孔钢板,固定1、4、7、8、11、14孔)差异有统计学意义;垂直压缩时侧方压力-应变第6组(14孔钢板,固定1、2、7、8、13、14孔)差异有统计学意义;三点弯曲压缩时垂直方向应变多组差异有统计学意义。结论①长钢板垂直方向稳定性及抗扭转稳定性较好;②过多增加螺钉数量并不能增加垂直方向稳定性;③14孔钢板螺钉分布在1、2、7、8、13、14孔时抗扭转稳定性较好;④14孔钢板当螺钉分布为1、3、6、9、12、14孔时三点弯曲压缩稳定性较好。Objective To investigate the biomechanieal effect of tibial shaft, and provide biomechanical basis for minimallyinvasive plate osteosynthesis (MIPO). Methods Forty pieces of humid cadaver tibia of adult were used in this research, and randomized into 8 groups. The change was measured and SPSS 12.0 statistics software was used for analysis. Results In vertical compression, only group 4 (14-hole plate, fixed with screws in the 1,4,7,8,11,14 holes) had statistical difference in the vertical direction strain and only group 6 (14-hole plate, fixed with screws in the 1,2,7,8,13,14 holes) had statistical difference in the lateral direction strain. In 3-pionts bending compression, many groups had statistical differences. Conclusion ①Stability of longplate is better in the vertical direction'strain and touque. ②Redundant screws can not increase the stability of the vertical direction.③The anti-reverse stability of l g-hole plate is better which fixed with screws in the 1,2,7,8,13,14 holes. ④Yrhe anti-curve stability- of 14-hole plate is better which fixed with screws in the 1,3,6,9,12,14 holes.
