活血降糖饮对2型糖尿病大血管病变的影响

摘　　要：目的研究活血降糖饮对2型糖尿病患者颈动脉内-中膜厚度、粥样硬化斑块面积、粥样硬化斑块检出率的影响,寻求糖尿病大血管并发症的防治方法。方法共收集气阴两虚血瘀型2型糖尿病患者180例,随机分为对照组、治疗组各90例。对照组采用降压、降脂、降糖及抗血小板聚集等综合治疗。治疗组在此基础上,加服益气养阴活血中药汤剂活血降糖饮。疗程为6个月。入组及疗程结束时行颈动脉彩超检查,分析治疗前后颈动脉内-中膜厚度,粥样硬化斑块面积、粥样硬化斑块检出率的变化情况。结果两组患者治疗后,颈动脉内-中膜厚度、粥样硬化斑块检出率均有改善,差异有统计学意义(P<0.05)。与对照组相比,治疗组改善更明显,差异有统计学意义(P<0.05)。结论在综合治疗基础上加用活血降糖饮治疗,患者颈动脉内-中膜厚度、粥样硬化斑块面积明显下降,说明益气养阴活血法能延缓动脉粥样硬化的发生发展,可有效预防糖尿病大血管并发症。Objective To investigate the effect of Huoxue Jiangtang Yin on carotid arterial intima-media thickness, plaque area and atherosclerotic plaque rate of type 2 diabetic patients, and seek the prevention of diabetic macrovascular complications. Methods 180 type 2 diabetes mellitus patients of Qi and Yin deficiency and blood stasis were collected, and randomly divided into control group and treatment group with 90 patients in each group. The patients in control group were treated with comprehensive therapy including hypotensors, lipid regulating drugs, hypoglycemic agents and platelet aggregation inhibitors, while in treatment group, the comprehensive therapy was used and Huoxue Jiangtang Yin was added. All patients were treated for 6 months. At the beginning and the end of the treatment, carotid arterial intima-media thickness, plaque area and atherosclerotic plaque rate were examined by carotid artery color Doppler ultrasound to analyze the changes on the above indexes. Results Carotid arterial intima-media thickness and atherosclerotic plaque rate were significantly ameliorated after 6-month treatment compared to that before the treatment （P 0.05）. In addition, compared with the control group, the treatment group improved more obviously （P 0.05）. Conclusion When the comprehensive therapy was added with Huoxue Jiangtang Yin, the carotid arterial intima-media thickness and atherosclerotic plaque rate of patients were significantly ameliorated, indicating that the Yiqi, Yangyin and Huoxue method is effective in delaying the development of atherosclerosis and preventing diabetic macrovascular complications.
