胸腔镜探查在外伤性血气胸并多发伤诊治中的应用体会

摘　　要：目的探讨胸腔镜在多发伤并外伤性血气胸诊断及治疗中的应用体会。方法分析多发伤并胸部外伤中21例具有胸部探查指征的临床资料,并根据伤情情况决定行胸腔镜下探查、治疗或中转开胸手术。结果 21例患者均在胸腔镜下作出正确诊断,3例中转开胸手术,术后无并发症发生,均治愈出院。结论严重的多发伤可危及生命,行胸腔镜探查可用于胸部外伤性血气胸的诊断及治疗,具有创伤小,胸内手术处理简单化可减轻二次创伤。Objective To experience the diagnosis and treatment of traumatic hemopneumothorax complicated with multiple injury under thoracoscopic. Methods The clinical data of 21 patients were retrospectively analyzed, and the related treatments were given based on patients＇condition. Results 21 patients were given the correct diagnosis under the thoracoseopic and 3 cases were converted to thoracotomy with no postoperative complications. All the patients were cured and discharged. Conclusions Severe multiple trauma can be life - threatening, and thoracoscopic exploration can be used for the diagnosis and treatment of traumatic hemopneumothorax complicated with multiple injury, which has less trauma and easier thoracic surgical treatment to reduce the secondary trauma.
