肺结核患者特异性抗原反应性IP-10的表达特点

摘　　要：目的研究活动性肺结核和肺炎及原发性肺癌患者外周血单核细胞(PBMCs)经结核分枝杆菌特异性抗原肽刺激后干扰素-γ诱导蛋白-10(IP-10)的表达特点。方法 40例初治活动性肺结核患者,25位结核分枝杆菌潜伏感染者,30位肺炎及原发肺癌(常见肺病)患者,采集EDTA抗凝血,用结核分枝杆菌特异性抗原肽刺激PBMCs,采用FlowCytomix流式技术检测细胞培养液上清上中IP-10和IFN-γ的表达。结果肺结核患者的PBMCs经结核分枝杆菌特异性抗原肽刺激后,培养液上清中IP-10的水平显著升高,三组间差别显著(F=11.52,P<0.0001),活动期肺结核组显著高于潜伏感染组和常见肺病组,与IFN-γ的分泌显著相关(P<0.0001)。结论结核分枝杆菌特异性抗原肽刺激外周血单个核细胞产生的IP-10可能成为肺结核的鉴别诊断指标。Objective To study the expression features of IP-10 and interferon- y , stimulated by specific MTB antigen peptide in the peripheral blood mononuclear ceils （PBMCs） of active pulmonary tuberculosis, bacterial pneumonia and primary lung cancer. Methods 40 patients with active pulmonary tuberculosis, 25 healthy controls with positive T. Spot results and 30 patients with bacterial pneumonia and primary lung cancer were enrolled. It used specific MTB antigen to stimulate PBMCs and then to test the concentration of IP-10 and IFN-y, with Flowcytomix streaming technology on the basis of cell culture fluid supernatant. Results The concentration of IP-10 and IFN-y,was elevated significantly in PBMCs culture fluid supernatant stimulated by peptide. The differences of the related indexes among the three groups showed statistical significance （ F = 11.52, P 〈 0. 0001 ）. Conclusion IP-10 stimulated with Mtb-specific peptides could be one of the biomarker for TB differential diagnosis.
