经皮空心钉与空心钉加肌骨瓣治疗股骨颈骨折疗效对比

摘　　要：目的:比较经皮空心钉加压微创技术和空心钉加股方肌肌骨瓣,治疗青壮年股骨颈骨折的疗效。方法:用空心钉加股方肌肌骨瓣治疗青壮年GardenⅢ型股骨颈骨折18例(A组),经皮空心钉加压微创技术治疗青壮年GardenⅢ型股骨颈骨折23例(B组),术后6个月患肢开始逐渐负重,比较两组患者骨折愈合及股骨头坏死情况。结果:术后随访15～55个月,A组优6例,良10例,差2例,优良率88.9%;B组优7例,良14例,差2例,优良率91.3%。结论:采用经皮空心钉加压微创技术治疗青壮年GardenⅢ型股骨颈骨折,手术创伤小,可降低骨折不愈合与股骨头缺血坏死的发生率,优于空心钉加股方肌肌骨瓣治疗,晚负重可能是关键因素。Objective：to study the Minimally invasive treatment with percutaneous compressing cannulated screw and cannulated screw internal fixation with Muscle-Bone graft for the treatment of Garden type Ⅲ femoral neck fracture in Young adults,improve fracture healing,reduce complications of femoral head necrosis.Methods：the Minimally invasive treatment for the treatment of Garden type Ⅲ femoral neck fracture in Young adults 18 cases（group A）,Muscle-Bone graft for the treatment of Garden type Ⅲ femoral neck fracture in Young adults of 23 cases（group B）,limb begin to weight after postoperative in June,we compared the fracture healing and femoral head necrosis among the two.Results：Regular follow-up continued 15-55 months.group A：excellent 6 cases,good 10 cases,and poor 2cases,was 88.9%;Group B：excellent 7 cases,good 14 cases,and poor 2 cases,was 91.3%.Conclusion：the Minimally invasive treatment for the treatment of Garden type Ⅲ femoral neck fracture in Young adults,which can reduce operation trauma and effectively reduce the complications,such as,not fracture healing and femoral head necrosis,Muscle-Bone graft is a bit poor,weight later may be the key factor.
