摘 要:目的探讨2型糖尿病(2DM)及糖尿病肾病(DPN)患者尿电导率、尿比重、尿糖及血糖相关性,以供临床参考。方法同步测定395例2型糖尿病患者组和54例糖尿病肾病患者与对照组体检健康人群尿电导率、尿比重、尿糖及血糖,并进行比较分析,用SYSMEX公司生产的UF-1000i全自动尿分仪测定尿电导率,Uritest500型尿液干化学分析仪器测定尿比重和尿糖,全自动生化分析仪器测定空腹血糖,用专用统计软件分析各指标的相关性。结果糖尿病组及糖尿病肾病患者组尿电导率比正常对照组低,DN组比DM组下降更明显,差异有统计学意义(P<0.01),而尿比重(试纸法)与正常对照差异不明显,差异无统计学意义(P>0.05)。相关性结果显示尿电导率与尿比重、尿糖与血糖呈正相关(r=0.516和r=0.669,P<0.01)。结论 2型糖尿病患者尿电导率下降,联合检测尿糖与血糖在糖尿病诊断和治疗中更有意义,而电导率检测对糖尿病肾病肾脏浓缩稀释功能的改变有重要意义。Objective To investigate the relationship between blood glucose and urine conductivity, urine specific gravity, urine glucose in patients with type 2 diabetes mellitus (2DM) or diabetic nephropathy, to provide references for the clinic. Methods The glucose levels in sera from 395 patients with type II diabetic, 54 patients in clinic diabetic nephropathy(DN) and homologous normal controls were measured by fully automatic biochemical analyzer, then the comparison and correla- tion analysis were made between blood glucose and urine conductivity, urine specific gravity, urine glucose measured by fully automatic urine elements analyzer UF-1000i and Uritest urine dry chemical analyser respectively. Results Urine conductivity was lower in group DM and DN than that in normal control group and it was lower significantly in Group DN than that in Group DM(P 〈 0.01). But the difference of urine specific gravity not significant between the patients and controls (P 〉 0.05). The correlation analysis indicated that there was a positive correlation between urine conductivity and urine specific gravity, urine glucose and blood glucose(r = 0.516 and r = 0.669,P 〈 0.01). Conclusion Combined determination of urine glucose and blood glucose is more useful than that of single item in diagnosing and treating DM, and urine conductivity declined in patients with DM is very significant in assessment of the change of renal concentration and dilution function.
