摘 要:目的探讨甲状腺激素在多囊卵巢综合征(PCOS)患者中的分布情况。方法 112例育龄期PCOS患者作为观察组,100例健康育龄期女性作为对照组,比较两组患者甲状腺激素(TSH、T3、T4、FT3、FT4)的分布差异,同时根据有无甲状腺功能异常将观察组分为甲减组和非甲减组,比较两组患者PCOS主要症状发生情况。同时甲状腺激素与PCOS的主要症状指标进行线性回归。结果观察组中43.8%(49/112)的患者伴有甲状腺功能减退,对照组13.0%(13/100)的患者伴有甲状腺功能减退,两组比较差异有统计学意义(P<0.05)。观察组TSH、T3、T4、FT3、FT4水平均明显高于对照组,差异有统计学意义(P<0.05)。伴甲减的PCOS患者中伴高雄激素血症、高胰岛素血症、肥胖、月经紊乱者比例分别是65.3%、71.4%、42.9%、53.1%,均明显高于非甲减组(P<0.05)。线性回归分析结果显示:TSH、T4、FT4与PCOS患者的T呈正相关(r=0.612、0.473、0.581,P<0.05),TSH、T4、FT4与PCOS患者的FINS呈正相关(r=0.449、0.352、0.533,P<0.05)。结论 PCOS患者伴有明显的甲状腺激素分泌失常,甲状腺激素与多囊卵巢综合征之间有着密切的关系。Objective To explore the distribution of thyroid hormone in PCOS patients. Methods Selected 112 cases of child-bearing age patients with PCOS as observation group, 100 healthy women of reproductive age as a control group. Compared the distribution of the thyroid hormone (TSH, T3, T4, FT3, FT4) between the two groups. At the same time, the' observation group was divided into hypothyroidism group and non-hypothyroidism group according to the presence or absence of thyroid function abnormalities. The two groups were compared with the occurrence of the main symptoms of PCOS. And took the linear regression between the thyroid hormone and the main symptoms of PCOS. Results 43.8% (49/112) of patients in the observation group associated with hypothyroidism, 13.0% of the control group (13/100)of patients with hypothyroidism, the difference was statistically significant (P 〈 0.05). TSH, T3, T4, FI'3, FI'4 level of the observation group were significantly higher than that in the control group, the difference was statistically significant (P 〈 0.05). The linear regression analysis showed that TSH , T4 and FT4 had a positive correlation with the T in the PCOS patients (r = 0.612, 0.473, 0.581, P 〈 0.05). TSH,T4 and FT4 had a positive correlation with the FINS in the PCOS patients (r = 0.449, 0.352, 0.533, P 〈 0.05). Conclusion The PCOS patients have secretion disorders on thyroid hormone. It has a close relationship between thyroid hormone and polycystic ovary syndrome.
