摘 要:目的观察重叠综合征(OS)与阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者肺功能的变化差异。方法随机选择OS和OSAHS患者56例,将其分为OS组32例和OSAHS组24例,两组患者的性别构成比、年龄和体质量指数比较,差异无统计学意义(P>0.05)。所有患者均进行夜间多导睡眠图、静态肺功能及血气分析检查,并对各项相关指标进行统计学处理。结果睡眠监测指标:OS组最低动脉血氧饱和度(LSaO2)低于OSAHS组(P<0.05),最长呼吸暂停时间(Lat)、睡眠呼吸紊乱指数(AHI)高于OSAHS组(P<0.05)。血气分析指标:OS组动脉二氧化碳分压(PaCO2)高于OASHS组(P<0.05),而氧分压(PaO2)及动脉血氧饱和度(SaO2)低于OSAHS组(P<0.05)。静态肺功能指标:OS组患者用力肺活量(FVC)、一秒钟用力呼气容积(FEV1)占预计值百分比(FEV1%)、FEV1/FVC均低于OSAHS组(P<0.05)。结论 OS患者较OSAHS患者夜间缺氧更明显,日间肺功能损害更严重。Objective To explore the difference in pulmonary function changes between overlap syndrome(OS) and obstructive sleep apnea hypopnea syndrome(OSAHS). Methods 56 patients were randomly divided into two groups,the OS group (32 cases) and the OSAHS group (24 cases). There were no statistical differences in the age, sex constituent ratio and bodylmass index between the two groups(P〉0.05 ). All the 56 cases were monitored for polysomnography, static state pulmonary function tests and blood gas analysis. The results were analyzed statistically. Results The lowest SaO2 in the OS group was lower than that in the OSAHS group (P〈O.05). The longest apnea time (Lat)and apnea hypopnea index (AHI)in the OS group were higher than those in the OSAHS group. The arterial partial pressure of carbon dioxide (PaCO2) in the OS group was higher than that in the OSAHS group (P〈0.05). Partial pressure of oxygen (PaO2)and oxygen saturation (O2Sat)were lower than those in the OSAHS group (P〈 0.05). The static state pulmonary function test showed that the forced vital capacity (FVC), forced expiratory volume in 1 s of predicted(FEV1%)and FEV1/FVC in the OS group were lower than those in the OSAHS group(P〈0.05). Conclusion OS patients have more obvious hypoxemia in the night and more serious injury of the pulmonary function in the daytime than OSAHS patients.
