参竹心康汤对慢性心衰患者的临床疗效及血管内皮功能的影响

摘　　要：目的观察参竹心康汤对慢性心衰患者心功能与血管内皮功能的影响。方法将91例慢性心衰患者随机分为治疗组46例,对照组45例。对照组以西医常规治疗,治疗组在此基础上加服参竹心康汤,疗程1个月。以内皮素(ET)、降钙素基因相关肽(CGRP)、每分输出量(CO)、左室短轴缩短率(FS)、左室射血分数(LVEF)、舒张早期流速峰值(EPFV)/舒张晚期流速峰值(APFV)即E/A作为观察指标。结果两组间心衰疗效差异有统计学意义﹙P<0.05);两组中医证候疗效差异有显著统计学意义﹙P<0.01);反映血管内皮功能指标ET、CGRP两组治疗前后差异有统计学意义﹙P<0.05～0.01),组间疗效比较治疗组优于对照组(P<0.01);反映左室收缩功能的指标CO、FS、LVEF两组治疗前后差异有统计学意义(P<0.05～0.01),组间疗效比较治疗组优于对照组(P<0.05～0.01);反映左室舒张功能的指标E/A两组治疗前后差异有显著统计学意义(P<0.01),组间疗效比较治疗组优于对照组(P<0.01)。结论参竹心康汤可提高慢性心衰患者的临床疗效,改善血管内皮功能及收缩与舒张功能。Objective To observe the effects of Shenzhu Xinkang decoction on the heart func tion and vascular endothelium function of patients with chronic heart failure. Methods 91 patients with chronic heart failure were enrolled and randomly divided into the treatment group （n=46） and the control group （n=45）. All patients were managed with conventional western medicine therapy.In addition, the patients in treatment group were administered by Shenzhu Xinkang decoction. The in- dices of the patients were observed as follow： endothelin （ET）, calcitonin gene related peptide （CGRP）, minute volume（CO）,left ventricular fractional shortening（FS）, left ventricular ejection frac- tion（LVEF）, peak early diastolic velocity（EPFV）/late diastolic peak flow rate（APFV） or （E/A）. Re- suits The curative effects of heart failure between the two groups were statistically significant （P〈 0.05） and the improvement in TCM syndrome between them were also significantly different （P〈 0.01）. The expressions of ET and CGRP which were provided objective indices for vascular en dothelium function before and after treatment in both groups were statistical significance （P〈0.05 orP〈0.01 ）, the indices of treatment group were FS and LVEF which were provided objective after treatment in both groups were statistical superior to those of control group （P〈0.01）. The CO, indices for left ventricle systolic function before and Significance（P〈0.05 or P〈0.01 ）, the indices of treat- ment group were superior to those of control group （P〈0.05 or P〈0.01 ）. The E/A which were pro- vided objective index for right ventricle diastolic function between before and after treatment in both groups were statistical significance（P〈O.01）, the index of treatment group was superior to that of control group （P〈0.01）.
