分期手术结合固本消痰汤治疗晚期髋关节结核11例

摘　　要：目的探讨分析分期手术结合固本消痰汤治疗晚期髋关节结核的临床疗效。方法将11例髋关节结核患者采用分期手术结合固本消痰汤治疗．采用Harris评分观察患者治疗前后的疼痛、髋关节活动情况。结果经过6—24个月的随访，10例患者功能均恢复良好；1例患者术后1年拍术侧髋关节X线片发现假体周围骨质丢失较多。给予对症处理及用药6个月，未发现骨质丢失加重现象。未见结核复发，无髋关节翻修病例，患者预后基本满意。Harris评分平均88分。结论通过系统的抗结核治疗和全面的术前评估，分期手术结合固本消痰汤治疗晚期髋关节结核是一种较安全和理想的手段。Objective To study the rapeutic efficacy of staged surgery and Guben Xiaotan De- coction for end-stage hip tuberculosis. Methods 11 cases of end-stage hip tuberculosis patients were treated with staged surgery and Guben Xiaotan Decoction. Harris Score was observed to eval- uated pain and hip-joint mobility before and after treatment. Results With 6～24 months follow-up, 10 cases had good functional recovery with no TB recurrence and resurgence, and the prognosis of patients was generally satisfied. The average of Harris Score was 88. Only 1 case was observed bone loss around artificial joint through X-ray after one year, but the phenomenon ceased after 6 months symptom-based treatments. Conclusion With the systematic anti-tuberculosis treatment and comprehensive preoperation assessment, staged surgery and Guben Xiaotan Decoction is a safe and ideal tratment for end-stage hip tuberculosis.
