摘 要:目的分析大理州结核杆菌培养药敏结果,了解肺结核病耐药情况及相关影响因素,为进一步完善耐药结核病控制策略提供决策依据。方法由县疾控中心对涂阳肺结核患者作痰标本采样、培养及流行病学调查调,由省疾控中心统一做药敏试验。结果获完整资料87人份,有耐药标本32人份,总耐药率为36.78%。其中:初治涂阳65份,耐药18份,初始耐药率27.69%,复治涂阳22份,耐药14份,获得性耐药率63.64%。结论大理州肺结核初始耐药率位于全国13.0%~42.1%的中位水平,获得性耐药率达全国27.5%~67.5%的高位水平。切实加强肺结核耐药监测、强化耐药病例治疗对国家防制结核病规划具有重要意义。Objective To analyze the drug sensitivity result of tubercle bacillus, understand the resistance of tu- berculosis and its relevant factors, and provide basis for further improving the control strategy of drug resistant tuberculosis. Method The Center for Disease Control and Prevention at county level sampled, cultured and did epidemiological investigation on sputum specimens of smear - positive tuberculosis patients, and the Center for Disease Control and Prevention at provincial level did unified drug sensitivity test. Results A total of 87 samples of complete data were obtained and 32 were drug resistant samples, with the total resistant rate of 36. 78%. Of which, 65 smear positive cases were initially treated and 18 were resistant to drugs, with the ini- tial resistant rate of 27.69% ; 22 smear positive cases were retreated and 14 were resistant to drugs, with the acquired resistant rate of 63.64%. Conclusions The initial resistance rate of tuberculosis in Dali was at the middle level, between 13.0% and 42. 1%, and the acquired resistance rate reached national high level, be- tween 27.5% and 67.5%. Strengthening the resistance monitoring of tuberculosis and treatment of resistant cases had important significance for national planning of control of tuberculosis.
