摘  要:目的比较不同机械通气模式对COPD呼吸衰竭患者肺功能及氧代谢的影响。方法 2010年7月一2012年6月收治COPD呼吸衰竭患者48例,随机分为A组和B组各24例。A组采用BiPAP模式进行治疗,B组则采用SIMV+PSV模式进行治疗,将2组治疗前及治疗后3 d、7 d的肺功能、血气分析及氧代谢指标进行比较。结果与治疗前比较,2组治疗后3 d、7 d的肺功能、血气分析及氧代谢指标均显著改善,且改善幅度B组均大于A组,差异有统计学意义(P均<0.05)。结论 BiPAP模式和SIMV+PSV模式均可显著改善COPD呼吸衰竭患者肺功能及氧代谢指标,而SIMV+PSV模式效果更佳。Objective To compare the influence of different mechanical ventilation mode for the pulmonary function and oxygen metabolism of patients with COPD combined with respiratory failure.Methods 48 patients with COPD combined with respiratory failure from July 2010 to June 2012 were selected as research object,and they were randomly divided into group A and group B with 24 cases in each group,group A were treated with BiPAP mode,group B were treated with SIMV + PSV mode,then the pulmonary function,blood gas analysis and oxygen metabolism indexes of two groups before and alter the treatment at third and seventh day were compared.Results Compared with those before treatment,the pulmonary function, blood gas analysis and oxygen metabolism indexes improved obviously in both groups at 3d,7d after treatment,and improvement in group B were better than those of group A(P 0.05).Conclusion Both BiPAP mode and SIMV + PSV mode can significantly improve the pulmonary function and oxygen metabolism indexes,and SIMV + PSV mode is the better one.
